百杏润肺化浊解毒汤对慢性持续期痰浊阻肺型支气管哮喘患者炎症因子及肺功能的影响

摘　　要：目的:观察百杏润肺化浊解毒汤对慢性持续期痰浊阻肺型支气管哮喘患者临床疗效的影响。方法:将60例慢性持续期痰浊阻肺型支气管哮喘患者随机分为两组,治疗组用百杏润肺化浊解毒汤治疗,对照组用孟鲁斯特钠片治疗,疗程均为4 w。治疗结束后观察两组临床疗效、咳嗽症状、中医证候积分变化、并检测治疗前后患者血中炎症介质白细胞介素-6(IL-6)、IL-8、肿瘤坏死因子-α(TNF-α)水平及肺功能指标FEV1、FVC、PEFR的变化。结果:治疗组和对照组总有效率分别为86.7%和76.7%,两组临床疗效比较无差异(P>0.05),但治疗组总有效率略高于对照组;治疗后两组患者日间、夜间咳嗽症状较治疗前均有明显改善(P<0.05);与治疗前比较,治疗后两组患者喘息、咯痰、胸闷、哮鸣音积分明显下降,IL-6、IL-8、TNF-α含量明显下降(P<0.05);两组患者FEV1、FVC、PEFR均有显著提高(P<0.05)。治疗组喘息、胸闷、哮鸣音症状较对照组改善明显(P<0.05)。结论:百杏润肺化浊解毒汤治疗慢性持续期痰浊阻肺型支气管哮喘有明显疗效,其作用机理可能是降低炎性因子分泌、改善患者肺功能以减轻患者的炎性反应,从而改善症状。Objective:To observe the clinical effect of Baixing Runfei Huazhuo Jiedu Decoction(百杏润肺化浊解毒汤)on chronic persistent bronchial asthma(CPBA)of Tanzhuozufei(痰浊阻肺)syndrome.Methods:A total of 60 CPBA cases of Tanzhuozufei(痰浊阻肺)syndrome were randomized into the treatment group and the control group.The treatment group was treated with Baixing Runfei Huazhuo Jiedu Decoction(百杏润肺化浊解毒汤)and the control group with Montelukast Sodium Tablets for 4 weeks.After treatment,the clinical efficacy,cough symptoms,and traditional Chinese medicine(TCM)syndrome scores of the two groups were observed,and the levels of inflammatory mediator interleukin-6(IL-6),inflammatory mediator interleukin-8(IL-8),tumor necrosis factor-α(TNF-α),and pulmonary function indexes[forced expiratory volume in one second(FEV1),forced vital capacity(FVC),and peak expiratory flow rate(PEFR)]were detected before and after the treatment.Results:The total effective rate of the treatment group and the control group was 86.7%and 76.7%,respectively,with no significant difference between the two groups(P>0.05).The cough symptoms at daytime and nighttime in both groups were significantly improved after treatment compared with those before treatment(P<0.05).Moreover,both groups registered significant alleviation of wheezing,expectoration,chest distress,and wheezing rale(P<0.05),decrease in content of IL-6,IL-8,and TNF-α(P<0.05),and rise of FEV1,FVC,and PEFR after treatment(P<0.05).The wheezing,chest distress,and wheezing rale were significantly relieved in treatment group compared with those in the control group(P<0.05).Conclusion:Baixing Runfei Huazhuo Jiedu Decoction(百杏润肺化浊解毒汤)exerts definite effect in the treatment of CPBA with Tanzhuozufei(痰浊阻肺)syndrome,by reducing the expression of inflammatory factors and improving the lung function of patients.
