摘 要:目的:观察重症感染患者替考拉宁血药谷浓度(Cmin)不达标的相关因素,为临床合理使用替考拉宁提供依据。方法:回顾性分析2018年5月~2019年3月入住我院重症监护室并监测替考拉宁Cmin的重症感染患者,根据替考拉宁Cmin的分布分为达标组(≥10μg·ml-1)和不达标组(<10μg·ml-1)。采集入组患者的性别、年龄、体质量、血肌酐(SCr)、肌酐清除率(Ccr)、血浆白蛋白信息进行Logistic回归分析,寻找替考拉宁Cmin不达标的影响因素。结果:监测替考拉宁Cmin共57例次其中达标16例次不达标41例次。Logistic分析结果显示,单次给药剂量为重症感染患者Cmin达标的影响因素(P<0.05)。单次给药剂量为800 mg的患者谷浓度达标率最高(52.9%)。结论:单次给药剂量对替考拉宁Cmin达标率有明显影响,建议监测替考拉宁Cmin,并根据Cmin调整给药剂量优化治疗方案。Objective:To observe the related factors in serum trough concentration(Cmin)of teicoplanin in patients with severe infection,so as to provide basis for the rational use of teicoplanin in clinical practice.Methods:Retrospective analysis was performed on the patients with severe infection admitted to the intensive care unit of our hospital for Cmin monitoring of teicoplanin from May 2018 to March 2019.According to the distribution of the Cmin of teicoplanin,the patients were divided into the standard group(≥10μg·ml-1)and the substandard group<10μg·ml-1).The gender,age,body weight,serum creatinine(SCr),creatinine clearance rate(Ccr)and plasma albumin during the teicoplanin treatment were collected for logistic regression analysis to find the possible factors influencing the substandard Cmin of teicolanine.Results:A total of 57 cases were monitored for Cmin including 16 cases up to standard and41 cases not up to standard.Logistic analysis results showed that single dose was the influencing factor in the concentration of teicolanine in the patients with severe infection(P<0.05).The patients with a single dose of 800 mg had the highest compliance rate(52.9%).Conclusion:Single dose is the factor influencing the Cmin compliance rate of teicolanine.It is suggested to monitor the Cminof teicolanine,and adjust the dose according to the Cmin to optimize the treatment plan.
