摘 要:目的评估肺结核空洞和肺真菌病空洞鉴别诊断中使用多层螺旋CT的应用效果。方法选择四川省宣汉县人民医院在2020年6月-2021年6月期间接诊的55例肺结核空洞患者和55例肺真菌病空洞患者,两组均接受多层螺旋CT诊断,分析多层螺旋CT在肺结核空洞和肺真菌病空洞的鉴别诊断效果。结果肺结核空洞患者的结节(23.64%)、毛刺(29.09%)、分叶(34.55%)、气液平面(32.73%)、内壁光滑(29.09%)、外壁毛糙(29.09%)比例小于肺真菌病空洞患者(54.55%、58.18%、67.27%、63.64%、63.64%、69.09%),肺结核空洞患者的钙化(60.00%)、卫星灶比例(72.73%)高于肺真菌病空洞患者(23.64%、30.91%),差异性具有统计学意义(P<0.05)。肺结核空洞患者炎症(52.73%)、纵膈淋巴结钙化(36.36%)、纵膈淋巴结肿大(29.09%)、胸膜凹陷比例(40.00%)高于肺真菌病空洞患者(18.18%、12.73%、7.27%、14.55%),差异具有统计学意义(P<0.05)。肺结核空洞患者和肺真菌病空洞患者的胸膜粘连率相比(81.82%、74.55%),差异无统计学意义(P>0.05)。结论在肺结核空洞与肺真菌病空洞的鉴别诊断中,两种疾病引起的空洞存在一定差异,采用多层螺旋CT可为临床提供参考依据,值得临床推广。Objective To evaluate the application effect of multi-slice spiral CT in the differential diagnosis of pulmonary tuberculosis cavity and pulmonary mycosis cavity.Methods 55 patients with pulmonary tuberculosis cavity and 55 patients with pulmonary mycosis cavity received in our hospital from June 2020 to June 2021 were selected.Both groups were diagnosed by multi-slice spiral CT.The effect of multi-slice spiral CT in the differential diagnosis of pulmonary tuberculosis cavity and pulmonary mycosis cavity was analyzed.Results The proportion of nodules(23.64%),burr(29.09%),lobulation(34.55%),gas-liquid level(32.73%),smooth inner wall(29.09%),rough outer wall(29.09%)in patients with pulmonary tuberculosis cavity was less than that in patients with pulmonary mycosis cavity(54.55%,58.18%,67.27%,63.64%,63.64%,69.09%),and the proportion of calcification(60.00%)and satellite focus(72.73%)in patients with pulmonary tuberculosis cavity was higher than that in patients with pulmonary mycosis cavity(23.64%,30.91%).The difference was statistically significant(P<0.05).The inflammation(52.73%),mediastinal lymph node calcification(36.36%),mediastinal lymph node enlargement(29.09%)and pleural depression(40.00%)in patients with pulmonary tuberculosis cavity were higher than those in patients with pulmonary mycosis cavity(18.18%,12.73%,7.27%and 14.55%)(P<0.05).There was no significant difference in pleural adhesion rate between patients with pulmonary tuberculosis cavity and patients with pulmonary mycosis cavity(81.82%,74.55%)(P>0.05).Conclusion In the differential diagnosis of pulmonary tuberculosis cavity and pulmonary mycosis cavity,multi-slice spiral CT can provide reference basis for clinical practice,which is worthy of clinical promotion.
