全髋关节置换术治疗创伤性股骨头缺血性坏死的临床研究

摘　　要：目的分析全髋关节置换术治疗创伤性股骨头缺血性坏死的临床研究。方法将2018年1月-2021年6月我院收治的60例创伤性股骨头缺血性坏死患者纳入研究对象,随机将患者分为观察组和对照组,每组30例,对照组患者采用闭合复位螺纹钉内固定治疗,观察组采用全髋关节置换术治疗,对两组患者总体治疗效果、生活质量评分、并发症发生率及治疗前后髋关节功能评分、疼痛评分进行对比分析,生存质量指标包括心理领域、环境领域、生理领域、社会领域。结果观察组治疗显效率和总有效率均明显高于对照组(P<0.05),不良反应发生率明显低于对照组(P<0.05),且观察组患者在心理领域、环境领域、生理领域、社会领域评分显著高于对照组(P<0.05);治疗后髋关节功能评分、疼痛评分水平均明显改善(P<0.05),且观察组改善水平明显高于对照组(P<0.05)。结论全髋关节置换术治疗创伤性股骨头缺血性坏死效果显著,能有效改善患者的整体疗效,减少并发症发生,降低患者的疼痛程度,提升患者的髋关节功能及生活质量。Objective To analyze the clinical research of total hip replacement in the treatment of traumatic avascular necrosis of the femoral head.Methods From January 2018 to June 2021,60 patients with traumatic avascular necrosis of the femoral head were included in the research subjects.The patients were randomly divided into observation group and control group,30 cases in each group,the patients in the control group were treated with closed reduction and screw internal fixation,and the observation group was treated with total hip replacement.The overall treatment effect,quality of life score,complication rate,hip function score before and after treatment,and pain score were compared and analyzed between the two groups.The quality of life indicators include the psychological field,the environmental field,the physiological field,and the social field.Results The treatment efficiency and total effective rate of the observation group were significantly higher than those of the control group(P<0.05),the incidence of adverse reactions was significantly lower than that of the control group(P<0.05),and the observation group was in the psychological,environmental,and physiological fields.The scores of domain and social domain were significantly higher than those of the control group(P<0.05).The hip joint function scores and pain scores were significantly improved after treatment(P<0.05),and the improvement level of the observation group was significantly higher than that of the control group(P<0.05).Conclusion Total hip arthroplasty is effective in treating traumatic avascular necrosis of the femoral head.It can effectively improve the overall curative effect of the patient,reduce complications,reduce the patient’s pain,and improve the patient’s hip joint function and quality of life.
