摘　　要：目的分析采用吻合器痔上黏膜环切钉合术与经肛门吻合器直肠切除术治疗Ⅳ度混合痔的临床疗效。方法选择2018年8月-2019年8月入本院肛肠科诊疗的104例Ⅳ度混合痔患者,随机分为对照组与研究组,均52例,对照组采取常规的经肛门吻合器直肠切除术治疗,研究组采取吻合器痔上黏膜环切钉合术治疗,对两组患者的临床效果进行统计分析,比较两组患者的并发症情况以及患者满意度。结果研究组较对照组治疗总有效率显著提高(P<0.01);研究组患者肛门狭窄、排便障碍、肛周疼痛以及肛门水肿等并发症评分较对照组明显降低(P<0.01);研究组较对照组患者的治疗满意度显著提高(P<0.01)。结论临床上采取吻合器痔上黏膜环切钉合术治疗Ⅳ度混合痔,术后患者并发症的评分降低,临床满意度高且效果显著,值得推广应用。Objective To analyze the clinical efficacy of stapled hemorrhoidopexy and transanal stapled rectal resection in the treatment of grade Ⅳ mixed hemorrhoids.Methods A total of 104 patients with gradeⅣmixed hemorrhoids admitted to the Department of Anorectal Surgery of our hospital from August 2018 to August 2019 were randomly divided into control group(n=52)treated with routine transanal stapled rectal resection and study group(n=52)treated with stapled hemorrhoidopexy.The clinical effects were statistically analyzed.The complications and patient satisfaction were compared between the two groups.Results The overall response rate was significantly higher in study group than in control group(P<0.01);the scores of complications such as anal stenosis,defecation disorder,pain and anal edema were significantly lower in study group than in control group(P<0.01);the treatment satisfaction was significantly higher in study group than in control group(P<0.01).Conclusion Stapled hemorrhoidopexy and stapling for the treatment of gradeⅳmixed hemorrhoids has the advantages of reduced postoperative complication score,high clinical satisfaction and significant effect.It is worthy of being widely popularized and applied.
