摘  要:目的探究外周血A20基因、NLRP3基因评价生物制剂治疗幼年特发性关节炎疗效的临床价值。方法选择2019年3月-2020年6月华中科技大学协和深圳医院收治的30例幼年特发性关节炎患儿设为病例组,另选同期儿童保健门诊30例无任何疾病的健康儿童设为健康组,比较两组外周血A20基因、NLRP3基因表达水平。同时对病例组患儿予以生物制剂治疗,比较治疗前后患者JADAS27评分、外周血A20基因、NLRP3基因表达水平的差异。并根据治疗效果将病例组患儿分为治疗有效亚组与治疗无效亚组,比较两亚组外周血A20基因、NLRP3基因表达水平。结果病例组患儿治疗前的外周血A20基因表达水平显著低于健康组,NLRP3基因表达高于健康组(P<0.05)。病例组患儿治疗后的外周血A20、NLRP3基因、JADAS27评分与治疗前相比差异有统计学意义(P<0.05)。治疗有效亚组的外周血A20高于治疗无效亚组,NLRP3基因低于治疗无效亚组(P<0.05)。经Pearson相关系数分析,JADAS27评分与外周血A20表达呈负相关,与NLRP3基因呈正相关(P<0.05)。结论应用生物制剂治疗幼年特发性关节炎后,进行外周血A20基因、NLRP3基因检测有助于监测幼年特发性关节炎病情活动及疗效。Objective To explore the clinical value of A20 gene and NLRP3 gene in peripheral blood to evaluate the efficacy of biological agents in the treatment of juvenile idiopathic arthritis.Methods 30 cases of children with juvenile idiopathic arthritis treated in Union Hospital of Huazhong University of science and Technology Shenzhen hospital from March 2019 to June 2020 were selected as the case group,and another 30 cases of healthy children without any disease in the same period were selected as the health group.The expression levels of A20 gene and NLRP3 gene in peripheral blood of the two groups were compared.At the same time,the children in the case group were treated with biological agents,and the differences of JADAS27 score,A20 gene and NLRP3 gene expression levels in peripheral blood were compared before and after treatment.In addition,according to the treatment effect,the patients were divided into treatment effective subgroup and treatment ineffective subgroup.The expression levels of A20 gene and NLRP3 gene in peripheral blood of the two subgroups were compared.Results The level of A20 gene expression in peripheral blood of children in the case group before treatment was significantly lower than the healthy group,and the expression of NLRP3 gene was higher than the healthy group(P<0.05).There were significant differences in A20,NLRP3 gene and JADAS27 scores of peripheral blood between the two groups after treatment(P<0.05).The A20 level in the effective subgroup was higher than the ineffective subgroup,and the NLRP3 gene level was lower than ineffective subgroup(P<0.05).Pearson correlation coefficient analysis showed that JADAS27 score was negatively correlated with A20 expression in peripheral blood and positively correlated with NLRP3 gene(P<0.05).Conclusion The detection of A20 gene and NLRP3 gene in peripheral blood is helpful to monitor the disease activity and curative effect of juvenile idiopathic arthritis.
