摘　　要：目的探讨髓过氧化物酶(MPO)-ANCA诊断ANCA相关血管炎(AAV)或其他疾病的临床意义。方法回顾性分析2017年7月至2019年6月青岛市市立医院检验科用酶联免疫吸附法(ELISA)检测初诊MPO-ANCA阳性住院患者共185例,其中33例MPO和PR3同时阳性,因此排除在分析之外。152例MPO-ANCA阳性患者中,确诊AAV患者41例(26.97%)设为AAV组;non-AAV患者111例(73.03%)设为non-AAV组。结果AAV患者41例(26.97%),以显微镜下多血管炎(MPA)患者为主;非AAV(non-AAV)患者111例(73.03%),肺部疾病患者占比最多,以肺间质纤维化伴感染为主;其次为脑卒中、肾脏疾病、结缔组织病、消化道疾病、恶性肿瘤等。MPO-ANCA诊断AAV的cut-off值是30.88U/mL,特异性是82.00%,敏感性是65.90%。结论MPO-ANCA阳性患者的临床疾病谱很广,对MPO-ANCA阳性结果的解读应结合临床表现、实验室检查等信息综合分析。Objective To explore clinical significance of myeloperoxidase(MPO)-ANCA in diagnosis of ANCA-related vasculitis(AAV)or other diseases.Methods Review and analyze 185 cases of newly diagnosed MPO-ANCA positive inpatients detected by enzyme-linked immunosorbent assay(ELISA)in Laboratory Department of Qingdao Municipal Hospital from July 2017 to June 2019,of which 33 cases were both positive for MPO and PR3,and were excluded from analysis.Among 152 cases MPO-ANCA-positive patients,41 cases(26.97%)were confirmed with AAV as AAV group;111 cases(73.03%)with non-AAV as non-AAV group.Results 41 cases(26.97%)of AAV patients were mainly microscopic polyangiitis(MPA)patients;111 cases(73.03%)of non-AAV(non-AAV)patients were mainly patients with pulmonary diseases,including lung interstitial fibrosis accompanied by infection;followed by stroke,kidney disease,connective tissue disease,digestive tract disease,malignant tumor and ect.Cut-off value of MPO-ANCA for diagnosis of AAV is 30.88 U/mL,specificity is 82.00%,and sensitivity is 65.90%.Conclusion Clinical disease spectrum of MPO-ANCA positive patients is broad,and interpretation of MPO-ANCA positive results should be combined with clinical manifestations,laboratory examinations and other information comprehensive analysis.
