比较有限切开内固定结合外固定支架与双切口双钢板内固定治疗复杂胫骨平台骨折的临床疗效

摘　　要：目的分析比较复杂胫骨平台骨折患者采用双切口双钢板内固定治疗与有效切开内固定结合外固定支架治疗的临床效果。方法将本院2015年11月至2018年11月期间收治的36例复杂胫骨平台骨折患者作为研究对象,按照患者的入院顺序,奇偶随机分组法,将其分为实验组与对照组,进行对比研究。将入院顺序为奇数的18例患者纳入实验组行有限切开内固定结合外固定支架治疗;将入院顺序为偶数的18例患者纳入对照组行双切口双钢板内固定治疗。对比两组患者的治疗效果。结果实验组与对照组患者膝关节功能评分无明显差异(P>0.05);实验组患者的心理健康评分显著高于对照组(P<0.05);实验组患者的社会情感职能评分显著高于对照组(P<0.05);实验组患者的躯体疼痛评分显著高于对照组(P<0.05);实验组患者的自理能力评分显著高于对照组(P<0.05);实验组患者的生活质量总评分显著高于对照组(P<0.05)。结论给予复杂胫骨平台骨折患者有限切开固定结合外固定支架治疗可显著提高患者的生活质量,且该手术方式在恢复患者膝关节功能上无明显差异,该治疗方式的临床应用价值显著,值得进一步推广使用。Objective The clinical effects of double incision and double plate internal fixation combined with external fixation stent were analyzed.Methods From November 2015 to November 2018,36 patients with complex tibia plateau fractures were divided into experimental group and control group according to the order of admission.18 patients with odd admission order were treated with limited incision internal fixation combined with external fixation stent,and 18 patients with even admission sequence were treated with double incision double plate internal fixation in the control group.The therapeutic effects of the two groups were compared.Results There was no significant difference in knee joint function score between the experimental group and the control group(P>0.05);mental health score of the experimental group was significantly higher than that of the control group(P<0.05);social emotional function score of the experimental group was significantly higher than that of the control group(P<0.05);body pain score of the experimental group was significantly higher than that of the control group(P<0.05);self-care ability score of the experimental group was significantly higher than that of the control group(P<0.05)The total score of quality of life in the experimental group was significantly higher than that in the control group(P<0.05).Conclusion The treatment of patients with complex tibial plateau fracture with limited incision and fixation combined with external fixation stent can significantly improve the quality of life of the patients,and there is no significant difference in the recovery of knee function in the patients.
