加温湿化高流量鼻导管通气(HHFNC)对婴幼儿急性低氧性呼吸衰竭(AHRF)的临床治疗效果分析

摘　　要：目的探讨婴幼儿急性低氧性呼吸衰竭(AHRF)患儿采用加温湿化高流量鼻导管通气(HHFNC)治疗效果。方法随机选取2018年4月至2019年6月本院接收的76例急性低氧性呼吸衰竭婴幼儿,按照入院单号、双号分为两组,各38例;对照组与研究组分别行低流量氧疗与加温湿化高流量鼻导管通气(HHFNC),统计各组转机械通气状况、呼吸支持及吸氧不良事件发生率,并记录吸氧与转归状况。结果在转机机械通气率、鼻粘膜损伤、浓痰及不耐受率方面,对照组分别是60.53%、26.32%、21.05%和26.32%,研究组分别是13.16%、5.26%、2.63%和7.89%,研究组相比于对照组更优(P<0.05)。在吸氧时间、低氧血症持续时间方面,研究组相比于对照组偏低(P<0.05);而住院时间比较,差异无统计学意义(P>0.05)。结论AHRF治疗过程中,HHFNC的应用,除降低转机机械通气率外,有助于不良事件发生率降低,缩短吸氧时间,确保良好的治疗效果。Objective To investigate the effect of warming humidification and high flow nasal catheter ventilation((HHFNC))in children with acute hypoxia respiratory failure((AHRF)).Methods 76 infants with acute hypoxia respiratory failure received from April 2018 to June 2019 were randomly divided into two groups according to admission single number and double number,38 cases in each group.The control group and the study group were treated with low flow oxygen therapy and warming humidification and high flow nasal catheter ventilation(HHFNC),to count the status of mechanical ventilation,respiratory support and the incidence of adverse events of oxygen inhalation,and to record the status of oxygen inhalation and prognosis.Results The mechanical ventilation rate,nasal mucous membrane injury,sputum and intolerant rate in the control group were 60.53%,26.32%,21.05%and 26.32%,respectively,and those in the study group were 13.16%,5.26%,2.63%and 7.89%,respectively,which were better than those in the control group(P<0.05).Compared with the control group,the duration of hypoxia in the study group was lower than that in the control group(P<0.05),but there was no significant difference in hospitalization time(P>0.05).Conclusion In the process of AHRF treatment,the application of HHFNC not only reduces the mechanical ventilation rate,but also helps to reduce the incidence of adverse events,shorten the oxygen absorption time and ensure a good therapeutic effect.
