螺旋CT三维重建(CT3D)及多平面重建(MPR)对胸腰段脊柱爆裂骨折诊断的优缺点及临床应用价值探究

摘　　要：目的对胸腰段脊柱爆裂骨折患者进行螺旋CT检查,采用三维重建(CT3D)和多平面重建(MPR)进行成像处理,讨论诊断方式的优缺点和临床价值。方法根据研究需要,将2017年1月1日至2019年12月31日我院收治的90例胸腰段爆裂骨折患者作为研究对象。对90例患者进行X平片、CT和螺旋CT三维重建和多平面重建进行检查和后续处理,探讨诊断效果和优缺点。结果在诊断结果方面,X平片、CT、CT3D和MPR都能够对椎体骨折进行显示。CT扫描在椎体中后柱骨折的诊断上具有显著优势,CT3D和MPR能够清晰的显示脊髓、神经根损伤。结论与X平片相比,CT在诊断爆裂骨折方面具有优势,可以显著区别单纯压缩骨折。CT3D和MPR不仅可以清晰显示患者胸腰段骨折状况,还能够显示椎管内部损伤,具有较好的临床应用价值。Objective Spiral CT examination was performed in patients with thoracolumbar spine burst fractures using three-dimensional reconstruction(CT3 D)and multi-plane reconstruction(MPR)imaging processing to discuss the advantages,disadvantages and clinical value of diagnostic modalities.Methods According to the needs of the study,90 patients with thoracolumbar burst fracture treated in our hospital from January 1,2017 to December 31,2019 were taken as the subjects.X-ray plain film,CT and spiral CT 3 D reconstruction and multiplanar reconstruction were performed in 90 patients,and the diagnostic effect,advantages and disadvantages were discussed.Results In terms of diagnostic results,X-ray plain film,CT,CT3 D and MPR can display vertebral fracture.Ct scan has significant advantages in the diagnosis of posterior column fracture of vertebral body.CT3 D and MPR can clearly show spinal cord and nerve root injury.Conclusion Compared with X-ray plain film,CT has advantages in the diagnosis of burst fracture and can significantly distinguish simple compression fracture.CT3 D and MPR can not only clearly show the condition of thoracolumbar fracture,but also show the internal injury of spinal canal,which has better clinical application value.
