摘 要:目的评估基于代谢热整合法的无创血糖仪在高海拔地区使用的性能。方法选取北京地区、西藏拉萨地区居住>5年的T2DM患者210例、395例,分别于空腹、餐后2 h使用无创血糖仪、实验室全自动生化分析仪(己糖激酶法)检测血糖,采用Parkes误差栅格、平均相对误差(MARD)、Bland-Altman分析检测方法配对血糖值的相关性,比较拉萨(平均海拔3650 m)与北京(平均海拔<100 m)多中心试验结果,评估无创血糖仪在高海拔地区的适用性。结果北京地区569个无创血糖仪与静脉血浆检测配对血糖值中535个(94.0%)落在一致性界限内,拉萨地区666个无创血糖仪与静脉血浆检测配对血糖值中624个(93.7%)落在一致性界限内,北京与拉萨地区无创血糖仪和静脉血浆血糖值的MARD为26.999%、24.817%,差异无统计学意义(P=0.141)。结论与平原地区多中心临床试验结果相比,无创血糖仪在海拔3650 m的准确性差异无统计意义,基于代谢热整合法的无创血糖仪可用于高海拔地区。Objective Evaluating the performance of noninvasive blood glucose monitor based on metabolic heat conformationat high altitude.Methods A total of 210 and 395 T2DM patients who have resided in Beijing and Lhasa,Tibet for more than 5 years respectively were enrolled in this study.Fasting blood glucose and 2-hour postprandial blood glucose(2 hPG)were tested in all the participants by the non-invasive glucose monitor,and Automatic laboratory biochemistry analyzer(hexokinase method)respectively.Parkes error grid analysis,MARD,Bland-Altman plots and regression analysis were used to compare the correlation between the two methods.The applicability of non-invasive blood glucose monitor in plateau region was studied by comparing the experimental results in Lhasa(3650 m)with the results of multi-center experiment in Beijing(less than 100 m).Results 535(94.0%)of 569 noninvasive blood glucose meters and venous plasma detection matching blood glucose values in Beijing fell within the consistent limit,624(93.7%)of 666 noninvasive blood glucose meters and venous plasma detection matching blood glucose values in Lhasa fell within the consistent limit,and the MARD of noninvasive blood glucose meters and venous plasma glucose values in Beijing and Lhasa were 26.999%and 24.817%,with no statistically significant difference(P=0.141).Conclusion The accuracy of non-invasive blood glucose meters in the area of 3650 m above sea level is the same as that in the plain area,suggesting that the non-invasive blood glucose monitor based on metabolic heat conformation can be applied in the high-altitude region.
