骨膜增生厚皮症合并慢性光化性皮炎1例

摘　　要：患者男,56岁,头面部手足皮肤增厚5年,加重伴面部红斑、丘疹、结节伴痒1年。皮肤科情况:面部、双耳皮肤可见浸润性红斑,其上可见散在结节、丘疹,皮肤油脂较多;前额皮肤可见皱褶及沟纹,头皮增厚呈回状,双手及双足可见杵状指/趾,双踝关节肿胀。X片提示长骨骨膜不规则增生,面部皮损组织病理符合慢性光化性皮炎。诊断:骨膜增生厚皮症合并慢性光化性皮炎。予以抗炎、抗组胺等治疗后部分皮损缓解。A 56-year-old male patient had a 5-year history of thickened skin on the head,face,hands and feet,and 1 year of aggravation accompanied by facial erythema,papules,nodules and itching.Dermatological examination showed that infiltrating erythema,nodules and papules were seen on the face,wrinkles and furrows were seen on the forehead skin,scalp thickened in a zigzag shape,fingers/toes were clubbed on both hands and feet,and edema present in both ankles.In addition,bulldog scalp,clubbing of fingers and toes and swelling of both ankle joints were seen.X-ray showed irregular periosteum hyperplasia of long bones.Histopathology of facial skin was consistent with chronic actinic dermatitis.Diagnosis:Primary hypertrophic osteoarthrosis with chronic actinic dermatitis.After anti-inflammatory and antihistamine treatment,part of the skin lesions healed.
