改良外剥内扎术联合心理支持疗法对环状混合痔患者的影响

摘　　要：目的探讨对环状混合痔患者采取改良外剥内扎术联合心理支持疗法干预对其术后并发症、心理状态的影响。方法选取2020年1月~2021年4月收治的70例环状混合痔患者使用随机数字表法分为实验组与对照组,各35例,两组患者均接受改良外剥内扎术治疗,对照组实施常规护理,实验组则实施常规护理联合心理支持疗法护理,比较各组术后并发症情况、术后疼痛程度及心理状态变化。正态计量资料采用t检验,计数资料采用x^(2)检验。结果实验组术后并发症总发生率5.72%,显著低于对照组的22.86%(P<0.05);两组患者术后2 h视觉模拟量表(VAS)评分对比无显著差异(P>0.05),术后24 h、48 h、72 h的VAS评分均逐渐下降,且实验组均显著低于对照组(P<0.05);术前两组患者焦虑自评量表(SAS)评分、抑郁自评量表(SDS)评分差异无统计学意义(P>0.05);实验组患者在手术当日与出院时SAS评分、SDS评分均显著低于对照组(P<0.05)。结论改良外剥内扎术联合心理支持疗法干预环状混合痔的效果较好,能显著缓解术后疼痛及降低术后并发症发生率,改善患者心理状态。Objective To investigate the effects of modified external stripping and internal ligation combined with psychological support therapy on the postoperative complications and psychological status of patients with cricoid mixed hemorrhoids.Methods 70 patients with cricoid mixed hemorrhoids admitted from January 2020 to April 2021 were selected and divided into the experimental group and the control group using the random number table method,with 35 patients each.The t test was used for normal measures and the x^(2)test was used for counting data.Results The overall incidence of postoperative complications in the experimental group was 5.72%,which was significantly lower than that in the control group,which was 22.86%(P<0.05);there was no significant difference in the visual analogue scale(VAS)scores between the two groups at 2 h after surgery(P>0.05),and the VAS scores gradually decreased at 24 h,48 h and 72 h after surgery,and the experimental group was significantly lower than the control group(P<0.05).There was no statistically significant difference in self-rating anxiety scale(SAS)and self-rating depression scale(SDS)scores between the two groups before surgery(P>0.05);SAS and SDS scores were significantly lower in the experimental group than in the control group on the day of surgery and at discharge(P<0.05).Conclusion Modified external peel and internal ligation combined with psychological support therapy intervention for cricoid mixed hemorrhoids was more effective,significantly relieving postoperative pain and reducing the incidence of postoperative complications,and improving patients’psychological status.
