心理护理对锁骨骨折钢板内固定患者围手术期的影响分析

摘　　要：目的分析心理护理对锁骨骨折钢板内固定患者围手术期的效果及心理状态改善的影响。方法选取本院2018年12月~2021年12月收治的80例接受钢板内固定术治疗的锁骨骨折患者,根据随机数字表法分为对照组和观察组,各40例。对照组患者接受常规护理,观察组患者在对照组基础上接受心理护理,两组均在护理一周后进行效果评价,对比两组汉密尔顿焦虑量表(HAMA)评分、汉密尔顿抑郁量表(HAMD)评分、数字等级评定量表(NRS)评分、中文版心理韧性量表(CD-RISC)评分、治疗依从性以及患者满意度。正态计量资料采用t检验,计数资料采用x^(2)检验。结果护理前,两组HAMA评分、HAMD评分、NRS评分及CD-RISC各维度评分均没有统计学差异(P>0.05);护理后,观察组HAMA评分、HAMD评分、NRS评分均显著低于对照组(P<0.05)。观察组CD-RISC各维度评分以及总评分均显著高于对照组(P<0.05)。观察组治疗依从性(95.00%)、患者满意度(92.50%)均显著高于对照组(70.00%、65.00%)(P<0.05)。结论心理护理可有效减轻锁骨骨折钢板内固定患者焦虑、抑郁等不良情绪,及时解决患者存在的心理问题,缓解疼痛感,提高心理韧性水平、治疗依从性,提高患者护理满意度,临床参考、推广以及借鉴价值较高,弥补了常规护理内容单一、方式被动等不足,更符合现代护理学需求。Objective To analyze the effect of psychological care on the perioperative effect and improvement of psychological status of patients with clavicle fracture with internal fixation of steel plates.Methods Eighty patients with clavicle fractures treated with internal fixation of steel plates admitted to our hospital from December 2018 to December 2021 were selected and divided into a control group and an observation group according to the random number table method,with 40 cases each.Patients in the control group received conventional care,and patients in the observation group received psychological care on the basis of the control group,and both groups were evaluated after one week of care,comparing the Hamilton anxiety scale(HAMA)score,Hamilton depression scale(HAMD)score,numerical rating scale(NRS)score,Chinese version of the Connor-Davidson resilience scale(CD-RISC)score,treatment compliance,and patient satisfaction.The t test was used for normal measures,and the x^(2)test was used for count data.Results Before care,there was no statistical difference between the HAMA score,HAMD score,NRS score and CD-RISC dimension scores of the two groups(P>0.05);after care,the HAMA score,HAMD score and NRS score of the observation group were significantly lower than those of the control group(P<0.05).The CD-RISC dimension scores and total scores of the observation group were significantly higher than those of the control group(P<0.05).The treatment compliance(95.00%)and patient satisfaction(92.50%)in the observation group were significantly higher than those in the control group(70.00%and 65.00%)(P<0.05).Conclusion Psychological care can effectively reduce anxiety and depression in patients with internal fixation of clavicle fracture plate,solve the psychological problems of patients in time,relieve pain,improve the level of psychological toughness,treatment compliance,and improve patient satisfaction with care,which has high clinical reference,promotion as well as reference value and makes up for the shortcomings of convent
