封闭管理模式对精神科护理人员心理状况的影响

摘　　要：目的探讨封闭管理模式对精神科护理人员心理状况的影响。方法选取2020年2月~2020年4月潍坊市精神卫生中心在封闭管理病房的护理人员(在岗)100例作为研究对象,使用焦虑自评量表(SAS)、抑郁自评量表(SDS)及症状自评量表(SCL-90)对100例(其中脱落5例)护理人员心理状况进行调查。正态计量资料实施t检验,计数资料实施x^(2)检验。结果依据SAS标准分,纳入研究的95例被试存在焦虑情绪的有38.95%:轻度者28.42%,中重度者10.53%;依据SDS标准分,纳入研究的护理人员中抑郁者有41.05%,其中轻度者29.47%,中重度者11.58%;封闭后护理人员的SAS、SDS评分显著高于封闭前(P<0.05);封闭后,护理人员SCL-90的抑郁、焦虑、恐怖、躯体化、强迫症状、人际关系敏感、敌对、偏执及精神病性因子评分均较封闭前显著升高,有统计学意义(P<0.05)。结论封闭管理模式下,精神科护理人员焦虑、抑郁等心理状态更显著,不但会对护理人员工作积极性造成影响,还会增加其心理问题隐患。为保证护理人员维持健康的心理状态,还需加强对封闭管理模式下护理人员的心理干预。Objective To explore the impact of hospital closed mode management on the psychological status of nursing staff.Methods A total of 100 nursing staff(on-the-job)in the closed management ward of Weifang Mental Health Center from February 2020 to April 2020 were selected as the research objects,the self-rating anxiety scale(SAS)and self-rating depression scale(SDS)and symptom check list 90(SCL-90)were used to investigate the psychological status of 100 nurses(including 5 cases dropped out).The t test was performed for normal measurement data,and the x^(2) test was performed for count data.Results According to the SAS standard score,38.95%of the 95 subjects included in the study had anxiety:28.42%were mild and 10.53%were moderately severe;according to the SDS standard score,among the nursing staff included in the study,41.05%were depressed,of which 29.47%were mild and 11.58%were moderate and severe;the SAS and SDS scores of the nursing staff after the closure were significantly higher than those before the closure(P<0.05);the scores of obsessive-compulsive symptoms,interpersonal sensitivity,hostility,paranoia and psychosis were significantly higher than those before the closure,with statistical significance(P<0.05).Conclusion Under the closed management mode,the anxiety,depression and other psychological states of psychiatric nursing staff are more significant,which will not only affect the enthusiasm of nursing staff,but also increase their hidden psychological problems.In order to ensure that the nursing staff maintain a healthy psychological state,it is necessary to strengthen the psychological intervention of the nursing staff under the closed management mode.
