摘 要:目的基于机器学习算法的静息态功能磁共振成像(rs-fMRI)技术,探讨rs-fMRI的三种功能指标(ALFF、fALFF、ReHo)作为三种特征对MRI阴性TLE患者的诊断价值。方法收集MRI阴性TLE患者113例,正常对照组68例,rsfMRI数据使用DPARSF进行处理,获得ALFF、fALFF、ReHo功能指标,再经PRoNTo软件分别进行诊断效能分析。结果在两组分类中,以ALFF指标作为分类标准的准确率为83.43%,AUC值为0.92;以fALFF指标作为分类标准的准确率为83.43%,AUC值为0.89;以ReHo指标作为分类标准的准确率为86.74%,AUC值为0.95(P=0.001)。基于三种rs-fMRI功能指标,MRI阴性TLE患者与对照组差异最大的脑区主要集中在右角回、双侧顶下缘角回、右楔前叶。结论MRI阴性TLE患者存在DMN相关脑区的异常改变,基于神经成像的计算机辅助方法使用ALFF、fALFF、ReHo三种功能指标能够识别MRI阴性TLE患者。Objective To explore the diagnostic value of three functional indicators(ALFF,fALFF,ReHo)of resting state functional magnetic resonance imaging(rs-fMRI)based on machine learning algorithms for MRI negative TLE patients.Methods A total of 113 patients with MRI negative TLE and 68 normal people as control group were collected.The rs-fMRI data were processed using DPARSF to obtain functional indicators of ALFF,fALFF,and ReHo.And then diagnostic efficacy was analyzed by PRoNTo software.Results In both groups,the accuracy of using ALFF index as a classification criterion was 83.43%with an AUC value of 0.92;the accuracy of using fALFF index as a classification criterion was 83.43%with an AUC value of 0.89;and the accuracy of using the ReHo indicator as the classification criterion was 86.74%with an AUC value of 0.95(P=0.001).Based on three rs-fMRI functional indicators,the brain regions with the greatest differences between MRI negative TLE patients and the control group were mainly concentrated in the right angular gyrus,bilateral inferior parietal marginal angular gyrus,and right anterior cuneiform lobe.Conclusion MRI negative TLE patients have abnormal changes in DMN-related brain regions.The computer-aided method based on neuroimaging can identify MRI negative TLE patients with ALFF,fALFF,and ReHo functional indicators.
