摘　　要：目的介绍外侧小切口交叉克氏针治疗GartlandⅢ型小儿肱骨髁上骨折的手术方法和疗效。方法采用外侧小切口交叉克氏针固定治疗儿童GartlandⅢ型肱骨髁上骨折43例,男33例,女10例;年龄2.6-12.4岁,平均5.58±2.23岁,术后石膏托固定3-4周。结果所有患儿术后均经过3-12月随访,平均7.47±2.66月,通过末次随访测量健肢和患肢Baumann角,使用配对t检验确定Baumann角进行比较,健肢平均72.88°±4.45°,患肢平均73.00°±4.40°,t=-0.58,P=0.565无差异性。经过末次随访Flynn’s评价标准均取得优良疗效。结论外侧小切口交叉克氏针治疗GartlandⅢ型小儿肱骨髁上骨折疗效良好。Objective To introduce the surgical method and efficacy of lateral small incision operation with cross Kirschner wire in the treatment of GartlandⅢsupracondylar fracture of humerus in children.Methods 43 children with Gartland TYPEⅢhumeral supracondylar fracture were treated by cross Kirschner wire with lateral small incision including 33 males and 10 females.The ages ranged from 2.6 to 12.4 years,with an average of 5.58±2.23 years old,which were fixed in plaster for 3-4 weeks after operation.Results All children were followed up for 3 to 12 months after operation,and the average follow-up time was 7.47±2.66 months.Baumann Angle of healthy and affected limbs was measured at the last follow-up,and the Baumann Angle was compared between healthy limbs and affected limbs by paired T test.The average Baumann Angle of healthy limbs was 72.88°±4.45°,and that of affected limbs was 73.00°±4.40°,there was no significant difference(t=-0.58,P=0.565).After the last follow-up,Flynn’s evaluation were all achieved excellent efficacy.Conclusion Cross Kirschner wire through lateral incision is effective in the treatment of Gartland TYPEⅢhumeral supracondylar fracture.
