摘 要:目的分析纤维支气管镜引导下PDT(经皮扩张气管切开术)的有效性及安全性。方法选定本院2017年3月-2021年3月住院治疗的142例气管切开患者,根据“治疗方法的不同”分组,参照组71例患者采纳常规气管切开术治疗,实验组71例患者采纳纤维支气管镜引导下PDT术治疗,比较两组手术指标、一次置管成功率、围术期并发症发生率、远期并发症发生率。结果实验组手术时间、颈部瘢痕长度均短于参照组,实验组术中出血量低于参照组,差异均具有统计学意义(P<0.05)。实验组一次置管成功率(98.59%)高于参照组(85.92%),实验组围术期并发症发生率(2.82%)、远期并发症发生率(1.41%)均低于参照组(15.49%、12.68%),差异均具有统计学意义(P<0.05)。结论纤维支气管镜引导下PDT术可有效缩短气管切开患者手术时间,提高一次置管成功率,降低围术期及远期并发症发生率。Objective To analyze the effectiveness and safety of PDT(percutaneous dilated tracheostomy)guided by fiberoptic bronchoscope.Methods 142 patients with tracheotomy in our hospital from March 2017 to March 2021 were selected and grouped according todifferent treatment methods.71 patients in the control group were treated with conventional tracheotomy,and 71 patients in the experimental group were treated with PDT under the guidance of fiberoptic bronchoscopy.The surgical indicators,the success rate of one-time catheterization,the incidence of perioperative complications,and the incidence of long-term complications were compared between the two groups.Results The operation time and neck scar length of the experimental group were shorter than those of the control group,and the intraoperative blood loss in the experimental group was lower than that of the control group.The difference was statistically significant(P<0.05).The one-time catheterization success rate(98.59%)of the experimental group was higher than that of the reference group(85.92%).The perioperative complication rate(2.82%)and the long-term complication rate(1.41%)of the experimental group were lower than those of the control group(15.49%,12.68%),the difference was statistically significant(P<0.05).Conclusion PDT under the guidance of fiberoptic bronchoscopy can effectively shorten the operation time of patients with tracheotomy,increase the success rate of one-time intubation,and reduce the incidence of perioperative and long-term complications.
