小儿慢性腹痛行血浆D-二聚体检测鉴别的临床价值分析

摘　　要：目的分析小儿慢性腹痛行血浆D-二聚体检测鉴别的临床价值。方法回顾性分析2018年10月至2020年2月于本院儿科治疗的158例慢性腹痛患儿的临床资料,所有患儿进行血、尿、便三大常规检查及腹部B超检查,同时检测肝肾功能、电解质、C-反应蛋白、血浆D-二聚体。记录所有患儿疾病分布情况及血浆D-二聚体检测结果。结果158例慢性腹痛患儿中,肠系膜淋巴结炎、急性胃炎、慢性胃炎、消化性溃疡、腹型过敏性紫癜、肠道蛔虫症、泌尿系结石、肠套叠、便秘分别为48例、35例、30例、24例、10例、5例、3例、2例、1例。158例慢性腹痛患儿中,8例患儿血浆D-二聚体水平升高,其中消化性溃疡2例,占消化性溃疡的8.33%;6例腹型过敏性紫癜,占腹型过敏性紫癜的60%。结论对于不明病因的慢性腹痛患儿若出现血浆D-二聚体水平上升,可能存在腹型过敏性紫癜的发生,需进一步全面检查,为临床诊断和治疗提供指导作用。Objective To analyze the clinical value of plasma D-dimer detection in children with chronic abdominal pain.Methods The clinical data of 158 children with chronic abdominal pain who were treated in our hospital from October 2018 to February 2020 were retrospectively analyzed.All children received blood,urine and stool routine examinations and abdominal B-ultrasound.Liver and kidney function,electrolyte,C-reactive protein and plasma D-dimer were also detected.The disease distribution and plasma D-dimer detection results of all children were recorded.Results Among 158 children with chronic abdominal pain,there were 48,35,30,24,10,5,3,2,and 1 cases undergoing with mesenteric lymphadenitis,acute gastritis,chronic gastritis,peptic ulcer,abdominal allergic purpura,intestinal ascariasis,urolithiasis,intusculum,and constipation respectively.Among 158 children with chronic abdominal pain,plasma D-dimer level was increased in 8 children,including 2 cases with peptic ulcer,accounting for 8.33%of peptic ulcer.There were 6 cases of abdominal allergic purpura,accounting for 60%.Conclusion For children with chronic abdominal pain of unknown etiology,if the level of plasma D-dimer increases,there may be the occurrence of abdominal allergic purpura,which requires further comprehensive examination to provide guidance for clinical diagnosis and treatment.
