摘 要:目的:探讨肥胖和代谢综合征的联合作用与中老年人群的糖尿病发病风险,为我国中老年人群糖尿病防治提供科学依据。方法:本研究为前瞻性队列研究,选用2011至2015年中国健康与养老追踪调查(CHARLS)数据。将研究对象根据体重和代谢状态分为代谢正常非超重/肥胖组、代谢正常超重/肥胖组、代谢异常/非超重肥胖组和代谢异常超重肥胖组。采用多因素Logistic回归模型评估肥胖、代谢综合征以及两者联合作用与糖尿病发病的关联性。结果:本研究共纳入4233名研究对象,平均年龄(58.94±8.76)岁;在为期4年的随访中,共有502人(11.86%)发生糖尿病。在调整性别、年龄、学历等混杂因素后,与代谢正常非超重/肥胖组相比,代谢异常超重/肥胖组显著增加了发生糖尿病风险(OR=4.15,95%CI 3.24~5.30)。结论:超重/肥胖和代谢综合征的联合作用会显著增加中老年人发生糖尿病的风险。Objective:To investigate the combined effects of obesity and metabolic syndrome on the risk of diabetes in middle-aged and elderly populations,and to provide a scientific basis for the prevention and treatment of diabetes in middle-aged and elderly populations in China.Methods:This study was a prospective cohort study,and data from the China Health and Retirement Longitudinal Study(CHARLS)2011—2015 were selected for participation.The subjects were divided into normal metabolic non-overweight/obese group,normal metabolic overweight/obese group,abnormal metabolic/non-overweight obese group and abnormal metabolic overweight/obese group according to the body weight and metabolic status.Multifactorial Logistic regression models were used to assess the association between obesity,metabolic syndrome,and the combined effect of both on the development of diabetes mellitus.Results:A total of 4233 subjects with a mean age of(58.94±8.76)years were included in this study;a total of 502(11.86%)developed diabetes during a 4-year follow-up.After adjusting for confounding factors such as sex,age,and education,the abnormal metabolic overweight/obese group had a significantly increased risk of developing diabetes compared with the normal metabolic non-overweight/obese group(OR=4.15,95%CI 3.24-5.30).Conclusion:The combined effect of overweight/obesity and metabolic syndrome significantly increases the risk of developing diabetes mellitus in middle-aged and older adults.
