床旁超声在急危重症医学中应用的调查研究

摘　　要：目的通过对二级以上医院急危重症医生床旁超声设备应用情况的调查,了解急危重症医学领域床旁超声技术的使用和远程诊治情况。方法采用自行设计问卷,以在线问卷星调查方式,通过急危重症医学领域微信群新媒介进行信息发放与收集。结果共调查医院466家,其中三级医院占78.5%,二级医院占21.5%家;98%以上调查对象认为床旁超声技术对急危重症临床实践有指导意义,30.7%受访医生能熟练将床旁超声应用于创伤监测和救治过程,完全没有接受培训学习者占28.1%;调查对象未能掌握并使用床旁超声技术主要原因为医院的不重视与不支持、科室未能配置设备;25.1%调查对象不了解远程超声技术,60.3%调查对象认为开展远程超声的困难是急危重症医生技术的缺乏。结论各级医院床旁超声拥有率、重视程度、培训和超声检查技术掌握率有待提升,应加快推进床旁超声技术在急危重症领域的应用,全面推进远程超声技术在急危重症医学领域的开展与应用。Objective Through the investigation on the application of point-of-care ultrasonography equipment of critical care doctors in hospitals of Grade II and above,the use of point-of-care ultrasonography technology and remote diagnosis and treatment in the field of critical care medicine were understood.Methods Using the method of new media technologies,466 valid questionnaires were investigated and analyzed by statistical method.Results A total of 466 hospitals were surveyed,of which 78.5%were levelⅢhospitals and 21.5%were level II hospitals;More than 98%of the respondents believed that bedside ultrasound technology had guiding significance in clinical practice of critical illness,30.7%of the doctors interviewed could use point-of-care ultrasonography skillfully in the process of trauma monitoring and treatment,and 28.1%had no training at all;The main reasons for the failure to master and use point-of-care ultrasonography technology were that the hospital did not attach importance to it and did not support it,and the department could not configure equipment,60.3%of the respondents believed that the difficulty of carrying out long-distance ultrasound was the lack of skills of emergency and critical care doctors.Conclusions The ownership rate of point-of-care ultrasono-graphy,the degree of attention,the training and the mastery rate of ultrasound examination in hospitals at all levels need to be improved,and the application of bedside ultrasound in the field of critical illness should be accelerated,overall promotion of the development and application of long-distance ultrasound technology in the field of emergency and critical care medicine.
