“一护一患”护理模式在高钾血症致严重心律失常患者行急诊血液透析中的应用

摘　　要：目的分析"一护一患"护理模式在高钾血症致严重心律失常患者采用急诊血液透析中的应用效果。方法随机选取中南大学湘雅医院急诊科2018年11月~2019年11月期间收治的高钾血症致严重心律失常患者共66例为研究对象,随机分为实验组(33例,采用急诊血液透析联合"一护一患"护理)与对照组(33例,采用急诊血液透析联合常规护理)。分析比对两组患者经过治疗与护理后血钾浓度变化情况、心电图变化情况、护理有效率、护理满意度、抑郁、焦虑自评量表评分变化情况。结果治疗前后组间差异均具有显著性,经过"一护一患"护理模式干预后,实验组患者较对照组患者血钾浓度、心电图电压下降效果更显著,心电图心律与T波形态有明显改善(P<0.05);实验组患者护理有效率、护理满意度、焦虑、抑郁自评量表评分均较对照组患者更佳,组间差异具备显著性(P<0.05)。结论高钾血症致严重心律失常患者实施急诊血液透析,在"一护一患"护理管理模式下,改善患者血钾浓度,缓解患者因高钾血症导致的严重心律失常,整体治疗效果更为显著,且"一护一患"护理管理模式将有效提升患者的护理满意度、有效率,降低其抑郁、焦虑情况,是一种具备较高实际应用价值的护理方法。Objective To analyze the effect of"one nurse one patient"nursing mode in emergency hemodialysis for patients with serious arrhythmia caused by hyperkalemia.Methods A total of66 patients with serious arrhythmia caused by hyperkalemia were randomly selected from the emergency department of Xiangya Hospital of Central South University from November 2018 to November 2019.All patients were randomly divided into two groups:the experimental group(33 cases,using emergency hemodialysis combined with"one nurse one patient"care)and the control group(33 cases,using emergency blood dialysis combined with"one nurse one patient"care)Dialysis combined with routine nursing.After treatment and nursing,the changes of blood potassium concentration,ECG,nursing efficiency,nursing satisfaction,depression and anxiety self rating scale were compared.Results The differences between the groups before and after treatment were significant.After intervention of the"one care,one disease"nursing model,the patients in the experimental group had a more significant reduction in blood potassium concentration and ECG voltage than the control group,and the ECG rhythm and T wave form were significantly improved(P<0.05),the self-assessment scales of nursing effectiveness,nursing satisfaction,anxiety and depression of the patients in the experimental group were better than those of the control group,and the differences between the groups were significant(P<0.05).Conclusions Under the mode of"one nurse one patient"nursing management,we can improve the patients’blood potassium concentration,relieve the patients’serious arrhythmia caused by hyperkalemia,the overall treatment effect is more significant,and the mode of"one nurse one patient"nursing management will effectively improve the patients’nursing satisfaction and efficiency,reduce their depression Anxiety is a kind of nursing method with high practical value.
