“H2H”营养管理模式对腹膜透析营养不良患者的效果研究

摘　　要：目的探讨"H2H"营养管理模式对腹膜透析营养不良患者营养状况的影响。方法采用便利抽样法抽取2019年1—7月在上海交通大学医学院附属第九人民医院透析中心行腹膜透析合并营养不良患者62例,采用随机数字法将患者分为干预组和对照组,各31例,对照组患者接受常规腹膜透析营养护理指导,干预组患者在常规护理的基础上,接受"H2H"营养管理模式指导。在干预前、干预3个月及6个月采用白蛋白与血红蛋白水平及SGA评分评估两组干预效果。结果干预后6个月干预组患者白蛋白与血红蛋白水平及均高于对照组(P<0.05),干预组患者SGA评分低于对照组(P<0.05)。结论"H2H"营养管理模式可改善腹膜透析患者营养不良状况。Objective To investigate the effect of"H2H"(Hospital to Home)nutrition management modelon the patients with malnutrition in peritoneal dialysis.Methods Using the convenience sampling method to extract 62 patients with peritoneal dialysis combined malnutrition in our hospital dialysis center from January 2019 to July 2019,patients were divided into intervention group and control group,31 patients respectively.Control group patients received conventional nutrition instruction,on the basis of conventional patients in intervention group accept"H2H"nutrition management model.Albumin,hemoglobin levels and SGA score of two groups were surveyed before intervention,3 months and 6 months after the intervention.Results After 6 months intervention,albumin and hemoglobin levels in the intervention group were higher than those in the control group(P<0.05),SGA score of patients in the intervention group was lower than control group(P<0.05).Conclusion"H2H"nutrition management model can improve the malnutrition status of peritonealdialysis.
