加巴喷丁辅助治疗颈肩及颜面部带状疱疹后神经痛及对T淋巴细胞的影响

摘　　要：目的分析加巴喷丁(gabapentin,GBP)辅助星状神经节阻滞(satellite ganglion blocks,SGB)治疗对颈肩及颜面部带状疱疹后神经痛(post-herpetic neuralgia,PHN)的应用效果及其对T淋巴细胞的影响。方法选取2016年5月—2017年8月在温州医科大学附属第一医院诊断且符合纳入标准的66例颈肩及颜面部PHN患者为研究对象,采用随机数字表法将其分为观察组和对照组,每组各33例,对照组行SGB治疗,观察组在对照组基础上加用GBP治疗,比较2组患者的临床疗效、疼痛程度、血浆β-内啡肽及T淋巴细胞亚群的水平。结果观察组患者的临床治疗总有效率明显高于对照组(P<0.05);治疗前,2组患者的疼痛视觉模拟(VAS)评分、血浆β-内啡肽及CD3^+、CD4^+、CD4^+/CD8^+水平差异均无统计学意义(均P>0.05);治疗后,2组患者的VAS评分明显低于治疗前(P<0.05),且观察组明显低于对照组(P<0.05),血浆β-内啡肽及CD3^+、CD4^+、CD4^+/CD8^+水平均明显高于治疗前(均P<0.05),且观察组明显高于对照组(均P<0.05)。结论GBP辅助SGB在治疗颈肩及颜面部PHN方面疗效显著,且可有效改善机体的T淋巴细胞水平,临床应用价值高。Objective To analyze the effect of gabapentin(GBP)assisted with satellite ganglion blocks(SGB)on post herpetic neuralgia(PHN)of neck,shoulder and facial shingles and its effect on T lymphocyte.Methods A total of 66 neck,shoulder and facial shingles PHN patients diagnosed in our hospital were selected from May 2016 to August 2017,who were randomly divided into observation group(n=33)and control group(n=33)according to the random number table method.The control group was treated with SGB,and the observation group was treated with GBP based on the control group.The clinical effect,pain degree,and the level of plasmaβ-endorphin,the level of T lymphocyte subsets were compared between the two groups.Results The total effective rate of clinical treatment in observation group was obviously higher than that of control group(P<0.05).Before treatment,there were no significantly differences in the score of visual analogue scale(VAS),the level of plasmaβ-endorphin,the level of CD3^+,CD4^+and CD4^+/CD8^+between the two groups(all P>0.05).After treatment,the VAS scores of two groups were obviously lower than that of before treatment(P<0.05),and the observation group was obviously lower than the control group(P<0.05).The level of plasmaβ-endorphin,CD3^+,CD4^+and CD4^+/CD8^+were obviously higher than those of before treatment(all P<0.05),and the observation group was obviously higher than the control group(all P<0.05).Conclusion The effect of GBP assisted with SGB is significant in the treatment of PHN of neck,shoulder and facial shingles,and it can improve the level of T lymphocyte,with high clinical application value.
