色素性痒疹二例皮肤镜特征及文献复习

摘　　要：目的报道2例色素性痒疹(prurigo pigmentosa,PP)患者的特征,归纳其皮肤镜表现,并复习相关文献进行总结。方法检索1971年—2021年国内外文献数据库,寻找PP皮肤镜学特征相关研究文献,结合解放军总医院第一医学中心患者分析总结PP在皮肤镜下可能的典型表现。结果共分析4例患者,包括目前文献检索到的2例和该文报道的2例患者。经分析总结出PP各个时期不同的表现。早期,淡红色背景下灶状分布点状血管、散在分布小片状黄褐色鳞屑及少量蜂窝状色素网。完全发展期,深红色背景下一致性分布的分支状的血管、卵圆形灶状分布的黄褐色鳞屑以及少量蜂窝状色素网。晚期,黄褐色背景下少量灶状分布的线状血管、少量散在分布黄褐色鳞屑及大片状弥漫分布的蜂窝状色素网。结论各个时期的PP均有较特异性的皮肤镜表现,可能与其组织病理特征相对应,可能对PP的非创伤性诊断提供依据,并能更好的与其他类型的红斑鳞屑性疾病进行鉴别。Objective To report the characteristics of 2 cases of prurigo pigmentosa(PP),summarize the dermatoscopic manifestations,and review relevant literature.Methods The domestic and foreign literature database from 1971 to 2021 was searched for the related research papers on dermoscopic characteristics of PP,and summarized the possible typical manifestations of PP under dermoscopy combined with the analysis of our cases.Results A total of 4 patients were analyzed,including two patients retrieved from the literature and two patients reported in this paper.The different manifestations of pigmented pruritus in different periods were summarized by analysis:in the early stage,focal blood vessels were distributed in light red background,scattered in small flaky yellow-brown scales and a small amount of cellular pigment net.In the full development stage,there were uniformly branching capillaries on a dark red background,ovoid focal yellow-brown scales,and a few cellular pigmented nets.In the late stage,there were a few focal linear capillaries,a few scattered yellow-brown scales,and a large diffuse honeycomb pigment net in the yellow-brown background.Conclusion PP in all stages has relatively specific dermatoscopic manifestations,which may correspond to its pathological features,may provide a basis for the non-traumatic diagnosis of pigmented prurigo,and can be better differentiated from other types of erythema and scaly diseases.
