血清KL-6在皮肌炎/多发性肌炎合并间质性肺病中的意义

摘　　要：目的探讨血清涎液化糖链抗原-6(KL-6)在皮肌炎/多发性肌炎(DM/PM)合并间质性肺病(ILD)中的诊断价值及临床相关性。方法收集2018年12月—2021年6月在新疆医科大学第一附属医院住院治疗并明确诊断为DM/PM的患者58例,其中合并ILD(DM/PM-ILD)的患者33例,无DM/PM-ILD(DM/PM-NILD)的患者25例,并收集同期住院治疗排除结缔组织病及ILD患者69例作为对照,记录患者症状、体征及血清KL-6水平。结果DM/PM-ILD患者的年龄与体质指数(BMI)均大于DM/PM-NILD患者;DM/PM-ILD组患者血清KL-6水平[1436.00(888.00,2087.00)U/ml]明显高于DM/PM-NILD组[320.00(202.00,579.50)U/ml]及对照组[170.00(145.50,218.50)U/ml]。有乏力、咳嗽咳痰、呼吸困难症状的患者血清KL-6水平均高于无症状患者。血清KL-6诊断DM/PM-ILD最佳临界值为865.5 U/ml,其灵敏度为78.8%,特异度为96.0%,约登指数为0.748,曲线下面积为0.932。结论血清KL-6是诊断DM/PM-ILD的重要血清学标志物,且与患者的临床症状有一定相关性,KL-6水平升高应警惕ILD的发生。Objective To investigate the diagnostic value and clinical relevance of serum Krebs von den Lungen-6(KL-6)in dermatomyositis/polymyositis(DM/PM)complicated with interstitial lung disease(ILD).Methods A total of 58 patients with DM/PM were diagnosed in the First Affiliated Hospital of Xinjiang Medical University from December 2018 to June 2021,including 33 patients with ILD(DM/PM-ILD)and 25 patients without ILD(DM/PM-NILD).69 patients with connective tissue disease and ILD were excluded during the same period.The symptoms,signs and serum KL-6 levels were recorded.Results The age and BMI of patients with DM/PM-ILD were higher than those with DM/PM-NILD.The serum KL-6 level of the DM/PM-ILD group[1436.00(888.00,2087.00)U/ml]was significantly higher than that of the DM/PM-NILD group[320.00(202.00,579.50)U/ml]and control group[170.00(145.50,218.50)U/ml].The serum KL-6 levels of patients with weakness,cough,expectoration and dyspnea were higher than those of asymptomatic patients.The optimal critical value of serum KL-6 in the diagnosis of DM/PM-ILD was 865.5U/mL,with a sensitivity of 78.8%,specificity of 96.0%,Yuden index of 0.748,and 0.932 below the curve.Conclusion Serum KL-6 is an important serological marker for the diagnosis of DM/PM ILD,and there is a certain correlation with the clinical symptoms of patients,the level of KL-6 should be vigilant against the occurrence of ILD.
