双叶皮瓣修复鼻尖肿物切除后软组织继发缺损45例临床分析

摘　　要：目的观察双叶皮瓣修复鼻尖部继发皮肤软组织缺损的临床疗效。方法2015年1月—2020年1月解放军总医院皮肤科门诊45例鼻尖肿物切除后继发皮肤软组织缺损患者,鼻尖部继发组织缺损面积约1.0 cm×1.0 cm~2.0 cm×2.0 cm大小。于缺损周围正常皮肤设计双叶皮瓣,用双叶瓣的小叶修复大叶缺损,将大叶旋转、推进修复鼻尖肿物切除后继发的组织缺损。结果45例患者术后皮瓣全部成活,仅1例术后切口轻度感染,经换药治疗后达Ⅰ期愈合,无皮瓣坏死等并发症发生。术后随访3~24个月,瘢痕轻微,鼻部形态完整,效果满意。结论双叶皮瓣是修复鼻尖肿物切除后继发缺损的理想方法,手术风险小,患者满意度高。Objective To observe the clinical effect of bilobed flaps on the repairing of the nasal tip soft tissue defect.Method From Jan 2015 to Jan 2020,forty-five cases of the bilobed flap were used to repair the nasal tip soft tissue defect after tumor resection,the range of defect from 1.0 cm×1.0 cm~2.0 cm×2.0 cm,bilobed flap designed around the normal skin nearby the defect,with the leaflet of bileaflet repairing the lobar defect,the leaf rotated and propulsed to repair of nasal tip tissue defects.Results Forty-five patients survived 100%of the bilobed flaps,and only one patient with light incision infection,after the dressing up to healing,with no flap necrosis and other complications.The postoperative follow-up ranged from 3 months to 24 months,with a slight scar,nasal morphology,and satisfactory effect.Conclusion Bilobed flap is an ideal method to repair the nasal tip defect after tumor resection of primary surgery.
