皮肤镜在头虱病和管型毛发鉴别诊断中的应用

摘　　要：目的总结头虱病和管型毛发的皮肤镜特征,探讨皮肤镜在二者鉴别诊断中的应用价值。方法收集2018年8月—2020年10月在西安医学院第一附属医院门诊就诊的头虱病和管型毛发各45例,对其行皮肤镜检查,对比各组皮肤镜征象的异同,从而得出皮肤镜征象对临床诊断的意义。结果头虱病和管型毛发在皮肤镜下有其各自特点。45例头虱病患者皮肤镜均发现虱卵。管型毛发患者皮肤镜皮损区见白色管状物均匀包绕毛发,且可沿毛干上下滑动。结论头虱病和管型毛发具有不同的镜下特征性表现,皮肤镜检查有助于临床中这两种疾病的诊断和鉴别诊断。Objective To describe the dermoscopic features of pediculosis captis and hair casts,and to investigate the application of dermoscopy in the diagnosis and differential diagnosis between pediculosis captis and hair casts.Methods Forty-five patients with pediculosis captis and hair casts were collected in the First Affiliated Hospital of Xi'an Medical College from August 2018 to October 2020.All the patients were given dermoscopic examinations,All dermoscopic data were collected and analyzed.Results Pediculosis captis and hair casts had their own characteristics under dermoscopy.Eggs were found in the dermascope of 45 patients with pediculosis capitis.Hair casts showed white amorphous masses encircling the hair shaft which were able to slide up and down.Conclusion Pediculosis captis and hair casts show different dermoscopic features,and dermatoscopy is helpful in the diagnosis and differential diagnosis of these two diseases.
