色素痣皮肤镜表现与组织病理分型的相关性研究

摘　　要：目的研究色素痣的皮肤镜表现与组织病理分型及其两者之间的相关性。方法收集2018年9月—2019年1月在天津医科大学总医院皮肤科门诊就诊,并经皮肤镜和组织病理学检查后确诊的色素痣患者143例。共有皮损172处,观察皮损的皮肤镜表现,并根据组织病理学特点进行分类,分析其皮肤镜表现与组织病理分型的对应关系。结果色素痣皮肤镜下主要表现为球状模式、网状模式、均质模式。色素痣根据痣细胞位置不同进行组织病理学分类,分别为皮内痣(92处)、混合痣(62处)、交界痣(18处)。其中皮内痣皮肤镜下主要模式为球状模式、均质模式,血管结构以弯曲血管最多见;混合痣皮肤镜下主要模式是球状模式,其内以规则线状血管、弯曲血管相对较多;交界痣皮肤镜下主要模式是网状模式,其内未见血管结构。结论色素痣的皮肤镜表现与组织病理分型存在一定的对应关系。Objective To search for the dermoscopic features and pathological types of melanocytic nevi and the relationship between the two.Methods A total of 172 lesions of 143 patients with melanocytic nevi were diagnosed by dermoscopy and histopathological examination from September 2018 to January 2019 in the dermatology clinic of the General Hospital of Tianjin Medical University.To observe the dermoscopic manifestations and pathological types of melanocytic nevi and analyze the corresponding relationship between them.Results Globular pattern,reticular pattern,and homogenous pattern were the main dermoscopic features of melanocytic nevi.Melanocytic nevi were classified into intradermal nevi(92 locations),compound nevi(62 locations)and junctional nevi(18 locations)according to the different location of nevus cells.Globular pattern and homogenous pattern were the main dermoscopic features of intradermal nevi and curved blood vessels were most common in intradermal nevi.The most common pattern of compound nevi was the globular pattern,and compound nevi have more regular linear vessels and curved blood vessels.The most common pattern of junctional nevi was the reticular pattern and there no vessel structure.Conclusion There is a certain corresponding relationship between dermoscopic manifestations and pathological types of melanocytic nevi.
