摘　　要：目的分析男性早发雄激素性脱发(androgenetic alopecia,AGA)的脂质谱,探讨其与脱发严重程度的相关性。方法病例组为男性早发AGA,对照组为年龄和性别匹配的健康受试者。分别比较病例组与对照组以及AGA严重程度间的常规脂质谱以及血浆致动脉粥样硬化指数(AIP)、小而密低密度脂蛋白(sdLDL)和载脂蛋白E(ApoE)基因型等。结果病例组总胆固醇(TC)、载脂蛋白A1(ApoA1)、sdLDL明显高于对照组(P<0.05);中重度AGA的sdLDL明显高于对照组(P<0.001);Spearman相关分析显示AGA严重程度与sd LDL(r_(s)=0.402,P=0.000)呈显著正相关;病例组与对照组以及AGA严重程度间Apo E基因型均无统计学差异(P>0.05);不同Apo E基因型间sd LDL无显著差异(P>0.05)。结论男性早发AGA与血脂水平存在相关性;sd LDL与AGA严重程度显著正相关,可能作为预测早发AGA患者高脂血症的血清标志物。Objective To analyze the lipid profile of early-onset androgenetic alopecia(AGA)in men and explore its correlation with the severity of alopecia.Methods Ninety males with early-onset AGA were selected with 68 age-and gender-matched healthy subjects as a control group.Conventional lipid profiles,small dense low-density lipoprotein(sdLDL)and aplipoprotein E(ApoE)genotypes were measured,and the atherogenic index of plasma was calculated for all cases and control subjects.Results Mean values of TC,ApoA1 and sdLDL were higher in cases than controls(P<0.05).The sdLDL of moderate and severe AGA was significantly higher than that of control groups(P<0.001).Spearman correlation analysis showed that the severity of AGA was significantly positively correlated with sdLDL(r_(s)=0.402,P=0.000).There were no significant differences in ApoE genes between case groups and control groups or between the severity of AGA(P>0.05).Conclusion There was a significant correlation between early-onset AGA and lipid levels in males.SdLDL is significantly positively correlated with the severity of AGA,and can be used as a serum marker to predict hyperlipidemia in early-onset AGA patients,so as to achieve the purpose of early diagnosis and prevent or delay the occurrence of cardiovascular diseases.
