妊娠晚期胎儿超声血流参数与胎儿结局相关性

摘　　要：目的探讨妊娠晚期胎儿超声血流参数与胎儿结局相关性。方法分析2017-01-01-2019-06-01河南科技大学第一附属医院60例胎儿不良结局孕产妇临床资料,根据不良程度分为轻度不良组36例与重度不良组24例,另选本院同期60名正常结局孕产妇为对照组,均于孕30~36周接受胎儿超声检查,并记录脐动脉与大脑中动脉血流参数,包括收缩期峰值流速与舒张末期流速比值(S/D)、搏动指数(PI)和阻力指数(RI)。采用单因素方差分析比较不同结局组脐动脉与大脑中动脉PI、S/D和RI值,采用多因素Logistic回归分析探究胎儿不良结局的影响因素。结果正常结局组胎儿脐动脉PI、S/D和RI值分别为0.83±0.19、2.34±0.45和0.57±0.08,轻度不良组分别为1.02±0.24、2.85±0.53和0.64±0.09,重度不良组分别为1.09±0.21、3.19±0.88和0.69±0.12,不同结局组胎儿脐动脉、大脑中动脉血流参数中PI、S/D和RI值比较差异有统计学意义(均P<0.05),正常结局组脐动脉PI、S/D和RI值均低于轻度不良组和重度不良组,均P<0.05。正常结局组、轻度不良组和重度不良组胎儿大脑中动脉PI分别为1.76±0.34、1.65±0.28和1.47±0.25,S/D分别为4.46±0.82、4.27±0.71和3.58±0.76,RI分别为0.83±0.12、0.78±0.18和0.61±0.15,各指标正常结局组、轻度不良组高于重度不良组(均P<0.05),正常结局组和轻度不良组差异无统计学意义,均P>0.05。多因素Logistic回归分析结果显示,脐动脉血流S/D值(OR=2.798,95%CI为1.280~6.117,P=0.010)是胎儿不良结局的危险因素,而大脑中动脉血流S/D值(OR=0.496,95%CI为0.269~0.912,P=0.024)是胎儿不良结局的保护因素。结论胎儿脐动脉、大脑中动脉血流参数与胎儿结局关联密切,在妊娠晚期对胎儿进行超声血流参数监测有助于预测胎儿结局。Objective To explore the correlation between fetal ultrasound blood flow parameters and fetal outcomes in late pregnancy.Methods The clinical data of 60 pregnant and lying-in women with adverse fetal outcomes in the First Affiliated Hospital of Henan University of Science and Technology from January 1,2017 to June 1,2019 were retrospectively analyzed.According to the degree of adverse outcomes,they were divided into mild adverse group(36 cases)and severe adverse group(24 cases).Another 60 pregnant and lying-in women with normal outcomes were selected as controls.All subjects received fetal ultrasound examination at 30-36 weeks of pregnancy,and blood flow parameters of umbilical artery and middle cerebral artery[peak systolic velocity/end-diastolic velocity(S/D),pulsatility index(PI),resistance index(RI)]were recorded.One-way analysis of variance was used to compare the PI,S/D and RI of umbilical artery and middle cerebral artery in different outcome groups,and multivariate Logistic regression analysis was used to explore the influencing factors of fetal adverse outcomes.Results The values of PI,S/D and RI of fetal umbilical artery in normal outcome group were 0.83±0.19,2.34±0.45 and 0.57±0.08,respectively,in mild adverse group were 1.02±0.24,2.85±0.53 and 0.64±0.09,respectively,and those in severe adverse group were 1.09±0.21,3.19±0.88 and 0.69±0.12,respectively.There were significant differences in PI,S/D and RI of fetal umbilical artery and middle cerebral artery blood flow parameters in different outcome groups(all P<0.05),The PI,S/D and RI of umbilical artery in normal outcome group were lower than those in mild adverse group and severe adverse group(all P<0.05).The PI of fetal middle cerebral artery in normal outcome group,mild adverse group and severe adverse group were 1.76±0.34,1.65±0.28 and 1.47±0.25,S/D were 4.46±0.82,4.27±0.71and 3.58±0.76,RI were 0.83±0.12,0.78±0.18and 0.61±0.15,respectively.The indexes in normal outcome group and mild adverse group were higher than those in severe a
