摘 要:目的探讨人参皂苷Rg1对重症急性胰腺炎(SAP)模型大鼠肺损伤的治疗作用及其可能的分子机制。方法SD大鼠75只,分为假手术组、模型对照组、乌司他丁组和人参皂苷Rg1低、高剂量组(10、40 mg·kg-1),每组15只。采用ELISA法检测大鼠血清脂肪酶活性和BALF炎症细胞因子IL-1β、IL-6、IL-18和TNF-α的水平;全自动生化分析仪检测血清淀粉酶活性;HE染色观察大鼠胰腺及左肺组织病理学改变;取右肺组织测肺组织W/D比;全自动血气分析检测仪检测动脉OI;qRT-PCR和Western blot法检测肺组织中炎症小体NLRP3、caspase-1、ASC的mRNA和蛋白表达水平。结果与假手术组比较,模型对照组血清脂肪酶和淀粉酶活性、胰腺组织和肺组织病理学评分,BALF中炎症细胞因子IL-1β、IL-6、IL-18和TNF-α的水平、肺组织W/D值、动脉血中OI值以及肺组织中炎症小体NLRP3、ASC和Caspase-1 mRNA和蛋白表达水平均显著升高(P<0.05);与模型对照组比较,人参皂苷Rg1高剂量组和乌司他丁组的上述检测指标水平均明显下降(P<0.05),人参皂苷Rg1低剂量组差异无统计学意义(P>0.05)。结论人参皂苷Rg1可能通过抑制炎症小体NLRP3的激活,降低肺组织炎症因子水平,有效地减轻急性胰腺炎大鼠肺损伤。Objective To investigate the therapeutic effect of Ginsenoside Rg1 on lung injury in rats with severe acute pancreatitis(SAP),and to study its possible molecular mechanism.Methods A total of 75 SD rats were divided into 5 groups(15 rats in each group):sham operated group,model control group,ulinastatin group,low dose group,and high dose group of ginsenoside Rg1(10,40 mg·kg-1).Except for the sham operation group,rats in all other groups were injected with 5%sodium taurocholate solution into the pancreaticobiliary duct to establish the SAP model.The rat serum lipase activity and bronchoalveolar lavage fluid(BALF)inflammatory cytokines interleukin(IL)-1β,IL-6,IL-18 and tumor necrosis factor(TNF)-αlevels were detected by enzyme-linkd immunosorbent assay(ELISA).Serum amylase activity was measured by automatic biochemical analyzer.The histopathological changes of rat pancreas and left lung were observed by hematoxylin-eosin(HE)staining.The wet/dry ratio(W/D)of right lung tissue was measured.The arterial oxygenation index(OI)was detected by automatic blood gas analyzer.The mRNA and protein expression of inflammasome NLRP3,caspase-1 and apoptosis-related speck-like protein(ASC)were detected by real-time quantitative PCR(qRT-PCR)and Western blot methods.Results Compared with the sham operation group,the model control group had significantly increasement in serum lipase and amylase activities,histopathological scores of pancreatic tissue and lung,levels of inflammatory cytokines IL-1β,IL-6,IL-18 and TNF-αin BALF,W/D value in lung tissue,OI value in arterial blood and expression levels of inflammasome NLRP3,ASC and caspase-1 mRNA and protein expression levels in lung tissue(P<0.05).Compared with the model control group,the above-mentioned detection index levels of the high-dose ginsenoside Rg1 group and the ulinastatin group were significantly decreased(P<0.05),and the difference in the low-dose ginsenoside Rg1 group was not statistically significant(P>0.05).Conclusion Ginsenoside Rg1 can reduce the level of inflammato
