基于中医阴阳理论探讨巨噬细胞极化在骨质疏松症中的作用

摘　　要：骨质疏松症(OP)是中老年人的常见病之一,自古以来就是学者关注的焦点。巨噬细胞极化是指当巨噬细胞受到微环境变化影响其发生的一个过程,极化形成的M1、M2型巨噬细胞影响骨代谢的平衡,对OP的治疗具有指导作用。巨噬细胞极化在骨稳态中的相互拮抗、转化,与中医阴阳理论的对立制约、相互转化、互根互用有相似的内涵。文章尝试从中医阴阳理论探讨巨噬细胞极化在OP中的作用,阐释当M1型巨噬细胞效应过强时,治以滋阴养阳、濡养生津;当M2型巨噬细胞效应不足时,治以温补阳气、以制阴盛。根据中医阴阳理论使巨噬细胞极化水平保持动态平衡调控骨稳态,可能是治疗OP的新思路。Osteoporosis,which has been the focus of scholars since ancient times,is one of the common diseases in middle-aged and elderly people.Macrophage polarization refers to a process in which macrophages are affected by microenvironmental changes.Polarized M1 and M2 macrophages affect the balance of bone metabolism and have a guiding role in the treatment of osteoporosis.Mutual antagonism and transformation of macrophage polarization in bone homeostasis have similar connotations with the contradiction,mutual transformation and mutual use of yin and yang theory in traditional Chinese medicine(TCM).The article explores the role of macrophage polarization in osteoporosis from the theory of yin and yang in TCM.When the effect of M1 macrophages exceeds M2 macrophages,nourishing yin and yang is necessary.On the contrary,warming the effect of yang to overwhelm yin is needed.According to the theory of yin and yang in TCM to make a balance of macrophage polarization,preventing and treating osteoporosis is achieved.To maintain the dynamic balance of macrophage polarization based on the theory of yin and yang in TCM to regulate bone homeostasis may be a new therapeutic method in the treatment of OP.
