肌骨超声在类风湿关节炎评估中的应用价值分析

摘　　要：目的分析肌骨超声在类风湿关节炎评估中的应用价值。方法选取该院自2018年6月—2019年9月收治的56例类风湿关节炎患者治疗前和达到DAS28缓解标准后的观察指标作为研究对象,观察患者治疗前后的肌骨超声图像,并对比患者治疗前后的关节内积液厚度、滑膜血流和骨侵蚀分布。结果从关节内积液厚度及滑膜厚度看,治疗后的滑膜厚度和关节内积液厚度分别为(3.64±0.06)mm和(2.95±0.09)mm,明显优于治疗前的(8.32±0.82)mm和(18.14±1.75)mm(t=40.2、63.6,P<0.05);治疗后的滑膜血流分布评分(51分)显著低于治疗前(117分)(χ~2=11.4,P<0.05);治疗达到DAS28缓解标准后患者骨侵蚀评分107分低于治疗前的118分,差异无统计学意义(χ~2=2.7,P>0.05)。结论应用肌骨超声检查类风湿关节炎患者,可有效观察到患者的早期病变情况,可以准确反映出患者关节滑膜厚度、关节腔积液、滑膜血流分布情况,对反映患者滑膜炎症的活动性评价具有较高敏感度,对为患者提供后续的精准治疗方式提供了佐证。Objective To analyze the application value of musculoskeletal ultrasound in the evaluation of rheumatoid arthritis.Methods56 patients with rheumatoid arthritis admitted to the hospital from June 2018 to September 2019 were treated with pre-treatment and DAS28 remission criteria.The musculoskeletal ultrasound images were observed before and after treatment.The intra-articular thick-ness,synovial blood flow and bone erosion distribution before and after treatment were compared.Results From the thickness of the intra-articular effusion and the thickness of the synovial membrane,the thickness of the synovial membrane and the thickness of the intra-articular fluid were(3.64±0.06)mm and(2.95±0.09)mm,respectively,which was significantly better than that before treatment(8.32±0.82)mm and(18.14±1.75)mm(t=40.2,63.6,P<0.05);the synovial blood flow distribution score after treatment(51 points)was significantly lower than before treatment(117 points)(χ~2=11.4,P<0.05);after the treatment reached DAS28,the score of 107 points with bone erosion score was lower than 118 points before treatment,the difference was not statistically significant(χ~2=2.7,P>0.05).Conclusion The application of musculoskeletal ultrasonography in patients with rheumatoid arthritis can effectively observe the early le-sions of patients,which can accurately reflect the synovial thickness,joint effusion and synovial blood flow distribution of patients,re-flecting the synovial membrane of patients.The activity evaluation of inflammation has a high sensitivity,which provides evidence for providing patients with follow-up precise treatment.
