泌尿排石合剂对肾结石体外冲击波碎石术后残石的排石疗效观察

摘　　要：目的:观察泌尿排石合剂对肾结石体外冲击波碎石(ESWL)术后残石的排石疗效。方法:将90例肾结石患者随机分成对照组和观察组,每组45例。两组患者均于入院当天行ESWL治疗,术后每日饮水2500~3000 ml并适当进行跳跃运动,对照组在此基础上口服盐酸坦索罗辛缓释胶囊,观察组在对照组治疗措施的基础上加用泌尿排石合剂,两组疗程均为4周。观察临床疗效及结石排出情况,比较临床症状积分、肾功能相关指标的变化情况,并进行安全性评价。结果:①观察组临床疗效优于对照组(P<0.05)。②治疗后,两组临床各症状积分减少(P<0.05),且观察组临床各症状积分少于对照组(P<0.05)。③观察组残石排净率高于对照组(P<0.05),排石时间、排净时间少于对照组(P<0.05),复发率低于对照组(P<0.05)。④治疗后,两组血肌酐和尿素氮水平降低(P<0.05),观察组血肌酐和尿素氮水平低于对照组(P<0.05)。⑤两组均无明显不良反应发生。结论:泌尿排石合剂可有效提高肾结石ESWL术后的结石清除率,缓解局部临床症状,提高患者肾功能,且安全性高。Objective:To observe the efficacy of Miniao Paishi Mixture on removing residual stones after extracorporeal shock wave lithotripsy(ESWL)of renal calculi.Methods:Ninety patients with renal calculi were randomly divided into the control group and observation group,45 cases in each group.Both groups were treated with ESWL on the day of admission.After operation,the patients drank 2500-3000 ml of water every day and do jumping exercise appropriately.The control group was treated with tamsulosin hydrochloride sustained-release capsule on this basis,and the observation group was treated with Miniao Paishi Mixture based on the treatment for the control group,with a course of 4 weeks.The clinical efficacy and the stone excretion were observed,the changes of clinical symptom scores and related indexes of renal function were compared,and the safety was evaluated.Results:①The clinical efficacy of the observation group was better than that of the control group(P<0.05).②After treatment,the clinical symptom scores of the two groups were decreased(P<0.05),and the clinical symptom scores of the observation group were less than those of the control group(P<0.05).③The removal rate of residual stone in the observation group was higher than that in the control group(P<0.05),the stone removal time and disappear time in the observation group were less than those in the control group(P<0.05),and the recurrence rate of the observation group was lower than that of the control group(P<0.05).④After treatment,the levels of serum creatinine and urea nitrogen in the two groups were decreased(P<0.05),and the levels of serum creatinine and urea nitrogen in the observation group were lower than those in the control group(P<0.05).⑤There were no obvious adverse reaction in both groups.Conclusion:Miniao Paishi Mixture can effectively improve the stone clearance rate after ESWL,alleviate local clinical symptoms,improve patients’renal function,and has high safety.
