心脏瓣膜置换术后妇产科手术的处理——附32例报告

摘　　要：本文报道32例心脏瓣膜置换术后妇产科手术的处理,本组病例包括置换机械瓣者28例,生物瓣者4例;置换二尖瓣29例,主动脉瓣2例,二尖瓣加主动脉瓣双瓣1例。年龄19~40岁,手术距换瓣时间为37天~6年。这些病例安全地度过了手术过程,未发生严重并发症。本文讨论了心脏瓣膜置换术后可能发生的并发症、抗凝剂的应用以及心脏瓣膜置换术后施行妇产科手术、麻醉的处理。The management of operation after cardiac valve replacement is a new problem in obstetric and gynecologic practice.Thirty-two such cases were operated upon in our hospital.Among them 29 had mitral valve prosthesis,2 aortic valve prosthesis and the other onewith mitral and aortic valve replacement.Twenty-eight patients had mechanical valveand 4 with tissue valve.The operations included subtotal hysterectomy,salpingooophorectomy,Cesarean section,tubal ligation,induced abortion,polypectomy of cervix.D&C,episiotomy,.vacuum extractor vaginal delivery and so on.The operations wag unevenful in these cases without any serious complication.The complication in thepatients with artificial valve,the use of anticoagulant and the management during obstetricand gynecologic operation are discussed.
