烧伤后蛋白质从外周到内脏的转移

摘  要:本文研究8例烧伤总面积60.3±19.7%男性成年病人,用N-甘氨酸示踪技术测定总体蛋白质代谢速度,用尿3-甲基组氨酸排出量估计肌肉蛋白质分解速度。结果示烧伤病人总体蛋白质的流量(Q)、合成(S)和分解速度(C)分别为9.20±1.54、6.10±0.77、7.07±0.97g/kg/d,相当于3名正常对照的2.0、1.9和2.3倍。烧伤病人尿3-甲基组氨酸排出量为8.40±2.22μmol/kg/d,相当于分解肌肉蛋白质2.00±0.54g/kg/d,此数值是4名正常对照的2.8倍。统计学分析表明总体蛋白质代谢的Q、S、C都与蛋白质摄入量(Ⅰ)有显著的正相关性;总体蛋白质分解与肌肉蛋白质分解之间也有显著的正相关性。Eight male adults with burn area 60±20%BSA were studied.N-glycine tracing technique was used to determine total body protein kinetics,and urinary 3-methylhistidine(3MH)excretion was measured to evaluate muscle protein catabolism.The results showed that the total body protein metabolic flux(Q),synthesis(S)and catabolism(C)in these patients were 9.20±1.54,6.10±0.77,7.07±0.97g/kg/d respectively,which were 2.0-,1.9-,2.3-fold those of 3 normal subjects.The urinary 3MH excretion in the burn patients was 8.40±2.22 mol/kg/d which was 2.8-fold that of 4 normal subjects.Statistic analyses showed that there were significant correlations between protein intake(Ⅰ)and Q,I and S,I and C,and C and muscle protein catabolism,but no correlation was found between I and net protein synthesis.The authors deem that the dramatic elevation of body protein metabolic rates indicates a peripheral to visceral protein transloation,The high protein intake thus provides both exogenous protein and endogenous amino acids through promoting protein translocation.
