基于程序逻辑模式视角下的社工参与糖尿病社区防控服务实践研究——以深圳社工试点项目为例

摘　　要：目的分析社工参与糖尿病社区防控服务成效,探索社工在社区慢性病管理中的作用发挥路径,对社工参与慢性病防控的有效性进行验证,进而对有效的服务方法进行标准化提炼,形成可复制经验。方法基于程序逻辑模式,对三年来社工参与糖尿病社区防控服务的资源投放、服务活动、成效等进行评估,对服务有效性进行持续性的验证。结果深圳市社工参与糖尿病防控试点项目采用卫社联动模式,社工运用社会学习等理论,联动社康机构的家庭医师团队,带动志愿者、企业商户等慈善资源,通过社区宣传、风险评估、患者自我管理小组、个案等综合干预提升糖尿病社区防控水平,服务成效良好。结论社工参与糖尿病社区防控服务有利于糖尿病患者的自我管理,有益于居民健康生活方式形成,为社工介入其他慢性病预防和管理工作提供了有益的先行经验。Objective Analyze the effectiveness of social workers’participation in diabetes prevention and control services in communities,explore the pathways for social workers to play a role in community chronic disease management,verify the effectiveness of social workers'participation in chronic disease prevention and control,and then standardize effective service methods to form replicable experiences.Methods Based on the program logic model,evaluate the resource allocation,service activities,and effectiveness of social workers’involvement in diabetes prevention and control services in communities over the past three years,continuously validating the effectiveness of these services.Results The Shenzhen social work pilot project for diabetes prevention and control adopts a health-social collaboration model.Social workers,applying theories such as social learning,collaborate with family doctor teams from community health institutions.They mobilize volunteers,business enterprises,and other charitable resources to enhance the level of diabetes prevention and control in communities through comprehensive interventions such as community promotion,risk assessment,patient self-management groups,and individual cases,achieving positive service outcomes.Conclusion Social workers’involvement in diabetes prevention and control services in communities benefits patients'self-management and contributes to the formation of healthy living styles among residents.It provides valuable pioneering experience for social work intervention in the prevention and management of other chronic diseases.
