河北省某医院体检3512例育龄女性生育选择及压力现状

摘　　要：目的分析调查河北省体检的育龄夫妇生育选择及生育相关压力现状。方法选取2019年1月至2020年12月在河北医科大学第二医院生殖医学科体检的已婚女性3512例作为研究对象,按照有无生育意愿分为有生育意愿组2885例和无生育意愿组627例,采用生育相关压力量表(FPI)评估生育压力,分析年龄、受教育程度、夫妻关系、婆媳关系、家庭人均月收入、户口、职业、夫妻类型、婚姻状况、家住类型、身体情况、是否有子女对河北省育龄夫妇生育选择、生育相关压力的影响,并采用二分类Logistic回归评估生育选择的危险因素。结果单因素分析结果显示,年龄、夫妻关系、婆媳关系、家庭人均月收入、户口、职业、夫妻类型、家住类型、身体情况是生育选择的影响因素(χ^(2)=293.486、57.562、21.619、24.150、55.434、167.625、30.897、363.966、48.487,P均<0.001);FPI评分无生育意愿组与有生育意愿组比较,差异有统计学意义(P<0.001);年龄、家庭人均月收入、职业、夫妇类型、家住类型、是否有子女是影响生育相关压力的因素(t/F=28.405、5.412、5.892、5.213、22.873、2.240,P均<0.001)。二分类Logistic回归分析结果显示,年龄大(P<0.001)、夫妻关系差(P<0.001)、婆媳关系差(P<0.001)、家庭人均月收入高(P<0.001)、农村户口(P<0.001)、职业农民(P<0.001)、非夫妻两人生活(P<0.001)、身体情况差(P<0.001)是与生育意愿有关联的影响因素。结论河北省育龄女性无生育意愿的原因主要与夫妻关系、婆媳关系、经济情况、夫妻个人情况、职业和户口等有关,生育压力主要来源于年龄、夫妻个人情况、经济情况和婆媳关系。Objective To analyze and investigate the current situation of fertility and fertility-related pressure in couples of childbearing age in Hebei Province.Methods A total of 3,512 married women who underwent physical examination in the Reproductive Medicine Department of the Second Hospital of Hebei Medical Uiversity were selected during Jan.2019 and Dec.2020 as the research objects.According to their willingness to bear children,they were divided into the willing group(n=2,885)and unwilling group(n=627).The Fertility Problem Inventory(FPI)was used to evaluate fertility pressure.The impacts of age,education level,husband-wife relationship,mother-in-law and daughter-in-law relationship,per capita monthly household income,household registration,occupation,family type,marital status,type of residence,physical condition,and children on the reproductive choices of couples were investigated.The risk factors of reproductive choices were assessed with binary Logistic regression.Results Univariate analysis showed age,husband-wife relationship,mother-in-law and daughter-in-law relationship,per capita monthly household income,household registration,occupation,family type,type of residence,and physical condition were the influencing factors of reproductive choices(χ^(2)=293.486,57.562,21.619,24.150,55.434,167.625,30.897,363.966,48.487,P<0.001).There was significant difference in FPI score between the two groups(P<0.001).Age,per capita monthly household income,occupation,family type,type of residence,and children were factors of fertility-related pressure(t/F=28.405,5.412,5.892,5.213,22.873,2.240,P<0.001).The result of binary Logistic regression analysis showed that,advanced age(P<0.001),poor husband-wife relationship(P<0.001),poor mother-in-law and daughter-in-law relationship(P<0.001),high monthly household income(P<0.001),rural household registration(P<0.001),farmer as occupation(P<0.001),non-husband-wife family type(P<0.001),poor physical condition(P<0.001)were independent influencing factors of willingness to bear children
