针灸联合表情肌功能康复训练治疗面神经麻痹的疗效及对面神经功能恢复的影响

摘　　要：目的:探讨针灸联合表情肌功能康复训练对面神经麻痹患者的疗效及面神经功能恢复的影响。方法:选择2016年1月~2021年3月安徽天长市中医院收治的60例面神经麻痹患者为研究对象,随机均分为对照组和观察组,对照组给予针灸治疗,观察组采用针灸联合表情肌功能康复训练,两组均治疗4周。比较两组临床疗效、治疗前后患侧面神经肌电检测[面神经复合肌肉动作电位(CMAP)波幅、瞬目反射R1潜伏期、肌电图运动单位束颤波发生率]、简易面神经功能评价量表(SFGS)评分及不良反应发生情况。结果:观察组治疗总有效率96.67%,高于对照组的80.00%(P<0.05);与治疗前比较,治疗后两组CMAP波幅均升高,且观察组高于对照组(P<0.05);与治疗前比较,治疗后两组R1潜伏期及束颤波发生率均降低,且观察组R1潜伏期短于对照组(P<0.05);治疗后两组束颤波发生率比较无统计学差异(P>0.05);与治疗前比较,治疗后两组SFGS量表中眼裂闭合评分、口角偏斜评分均降低,且观察组低于对照组(P<0.05)。治疗期间及治疗后两组均无明显不良反应发生。结论:针灸联合表情肌功能康复训练对面神经麻痹患者疗效显著,可明显改善患者的相关症状与体征,有效恢复患者面神经功能,且不会增加不良反应的发生。Objective To investigate the efficacy of acupuncture combined with expression muscle function rehabilitation in the treatment of patients with facial nerve palsy and the effect on the recovery of facial nerve function.Methods Sixty patients with facial nerve palsy admitted to the Tianchang Traditional Chinese hospital from January 2016 to March 2021 were selected and randomly divided equally into a control group and a study group,30 cases/group.The control group was given acupuncture treatment,and the study group was treated with acupuncture combined with expression muscle function rehabilitation.Both groups were treated for 4 weeks.The clinical efficacy,the affected facial nerve EMG tests[facial compound muscle action potential(CMAP)wave amplitude,transient reflex R1 latency,EMG motor unit bundle tremor wave incidence],the Simple Facial Nerve Function Evaluation Scale(SFGS)score and the occurrence of adverse effects were compared between the two groups before and after treatment.Results The total effective rate of treatment was 96.67%higher in the study group than 80.00%in the control group(P<0.05).Compared with the pre-treatment period,the CMAP wave amplitude increased in both groups,and the observation group was higher than the control group(P<0.05).Compared with the pre-treatment period,the incidence of R1 latency and fascicular fibrillation wave decreased in both groups after treatment,and the R1 latency of the study group was shorter than that of the control group(P<0.05),there was no statistical difference in the incidence of fascicular fibrillation wave between the two groups after treatment(P>0.05).Compared with the pre-treatment period,the ocular fissure closure score and orofacial deviation score in the SFGS scale were lower in both groups after treatment,and the study group was lower than the control group(P<0.05).No significant adverse reactions occurred in either group during or after treatment.Conclusion Acupuncture combined with expression muscle function rehabilitation training has a significant
