三虫半夏白术天麻汤联合丹参注射液对风痰瘀阻型急性缺血性脑卒中患者血液流变学及血清炎症指标的影响

摘　　要：目的:探讨三虫半夏白术天麻汤联合丹参注射液对风痰瘀阻型急性缺血性脑卒中(AIS)患者血液流变学及血清炎症指标的影响。方法:选取2019年1月~2020年12月合肥市第八人民医院收治的100例AIS患者,根据随机数字表法分为两组,对照组(n=50)接受丹参注射液治疗,观察组(n=50)接受三虫半夏白术天麻汤联合丹参注射液治疗,均连续治疗2周。比较两组患者临床疗效和治疗后中医症状积分、神经功能缺损程度、血液流变学指标、血清炎症指标及不良反应发生情况。结果:观察组临床治疗总有效率明显高于对照组(P<0.05);治疗后观察组中医症状积分、美国国立卫生研究院卒中量表(NIHSS)评分和高低切全血粘度、血浆粘度、纤维蛋白原及血清白介素-6(IL-6)、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)水平明显低于对照组(P<0.05);两组患者均未出现明显不良反应。结论:三虫半夏白术天麻汤联合丹参注射液治疗风痰瘀阻型AIS疗效显著,可有效改善血液流变学和血清炎症因子水平,提升神经功能,安全性高,值得临床推广。Objective:To investigate the effect of Sanchong Banxiabaizhu Tianma Decoction combined with Danshen Injection on blood rheology and serum inflammatory indexes for patients with wind-phlegm blood-stasis type acute ischemic stroke(AIS).Methods:A total of 100 AIS patients admitted to our hospital(January 2019 to December 2020)were selected and divided into two groups according to the random number table method.The controlled group(n=50)received Danshen Injection,and the observation group(n=50)received Sanchong Banxia Baizhu Tianma Decoction combined with Danshen Injection for 2 weeks.The clinical efficacy and TCM symptom scores,neurological deficits,blood rheology indexes,serum inflammation indexes and adverse reactions after treatment were compared between the two groups of patients.Results:The total effective rate of clinical treatment in the observation group was significantly higher than that in the controlled group(P<0.05).After treatment,the observation group’s TCM symptom score,the National Institutes of Health Stroke Scale(NIHSS)score,high-cut and low-cut whole blood viscosity,plasma viscosity,fibrinogen and serum interleukin-6(IL-6),high-sensitivity C-reactive protein(hs-CRP)and tumor necrosis factor-α(TNF-α)levels were significantly lower than those of the controlled group(P<0.05).There were no obvious adverse reactions in the two groups of patients.Conclusion:Sanchong Banxia Baizhu Tianma Decoction combined with Salvia Miltiorrhiza injection is effective in the treatment of AIS with wind-phlegm blood-stasis type,it can effectively improve blood rheology and serum inflammatory factor levels,enhance nerve function,has high safety,and is worthy of clinical promotion.
