自拟活血化瘀汤加味联合苏红汤熏洗治疗内痔嵌顿急性发作的临床研究

摘　　要：目的:观察自拟活血化瘀汤加味联合苏红汤熏洗对血瘀型内痔嵌顿急性发作患者临床疗效。方法:选取2020年1月~2022年1月于我院就诊的血瘀型内痔嵌顿急性发作患者74例,采用简单随机法分为研究组(n=37)、对照组(n=37)。对照组给予常规抗感染治疗联合苏红汤熏洗进行治疗,研究组在对照组的基础上联合自拟活血化瘀汤加味进行治疗,两组患者均治疗2周。比较两组患者治疗2周后临床疗效,记录其治疗前及治疗2周后中医症候积分、疼痛情况[疼痛视觉评分(VAS)、前列腺素E2(PGE2)、疼痛物质P(SP)]、炎性因子水平[白细胞介素-6(IL-6)、白细胞介素-1β(IL-1β)、干扰素-γ(IFN-γ)],比较两组患者临床症状恢复情况[内痔复位时间、疼痛消失时间、排便恢复时间]。结果:治疗2周后,研究组临床总有效率(86.49%)明显高于对照组(64.86%)(P<0.05)。治疗2周后,两组患者中医症候积分、疼痛情况、炎性因子水平均明显低于治疗前,且研究组明显低于对照组(P<0.05);治疗期间后,研究组内痔复位时间、疼痛消失时间、排便恢复时间均明显短于对照组(P<0.05)。结论:自拟活血化瘀汤加味联合苏红汤熏洗可有效改善血瘀型内痔嵌顿急性发作患者临床症候,降低患者疼痛感及炎症反应,具有较好的疗效。Objective:To explore the clinical efficacy of applying Modified Self-Made Huoxue Huayu Decoction combined with Suhong Decoction fumigation in the treatment of acute attack of internal hemorrhoid incarceration of blood stasis type.Methods:74 patients with acute attack of internal hemorrhoid incarceration of blood stasis type who were treated in the hospital from January2020 to January 2022 were selected and divided into a study group(n=37)and a controlled group(n=37)by simple randomization method.The controlled group was given conventional anti-infective therapy combined with Suhong Decoction fumigation,while the study group was combined with Modified Self-made Huoxue Huayu Decoction on the basis of the controlled group.Both groups were treated for 2 weeks.The clinical efficacy of the two groups of patients after 2 weeks of treatment was compared,and the TCM symptoms scores,pain status[Visual Analogue Scale(VAS),prostaglandin E2(PGE2),pain substance P(SP)]and inflammatory factors[interleukin-6(IL-6),interleukin-1β(IL-1β),interferon-γ(IFN-γ)]were recorded before treatment and after 2 weeks of treatment,and the recovery status of clinical symptoms[internal hemorrhoid reduction time,pain disappearance time,defecation recovery time]were compared between the two groups of patients.Results:After 2 weeks of treatment,the total clinical effective rate(86.49%)in the study group was significantly higher than that in the controlled group(64.86%)(P<0.05).The scores of TCM symptoms,pain status and levels of inflammatory factors in the two groups after 2 weeks of treatment were significantly lower than those before treatment,and the above indicators were significantly lower in the study group than those in the controlled group(P<0.05).The internal hemorrhoid reduction time,pain disappearance time and defecation recovery time during treatment were significantly shorter compared with those in the controlled group(P<0.05).Conclusion:Modified Self-made Huoxue Huayu Decoction combined with Suhong Decoction fumigation can effecti
