养阴益气复元汤加减治疗慢性肺源性心脏病(肺肾气虚型)疗效观察

摘　　要：目的:探究养阴益气复元汤加减治疗慢性肺源性心脏病(肺肾气虚型)的疗效。方法:将本院2019年6月~2021年5月收治的86例慢性肺源性心脏病(肺肾气虚型)患者采用随机数字表法分为观察组(43例)与对照组(43例)两组。对照组采用常规西医治疗,观察组在对照组基础上联合养阴益气复元汤加减治疗,均持续治疗2个月。对比两组患者治疗疗效。于治疗前后比较两组患者中医证候积分;采用肺功能检测仪检测患者肺功能指标[第1秒用力呼气容积(FEV_(1))、用力肺活量(FVC)和FEV_(1)/FVC];采用血气分析仪检测患者血氧分压(PaO_(2))、血氧饱和度(SaO_(2))和二氧化碳分压(PaCO_(2));记录两组患者治疗期间不良反应发生情况。结果:观察组患者治疗有效率为90.70%,显著高于对照组72.09%(P<0.05)。治疗后,观察组患者症候积分显著低于对照组(P<0.05);FEV_(1)、FVC和FVC/FEV_(1)等肺功能和PaO_(2)、SaO_(2)和PaCO_(2)等血气指标均显著高于对照组(P<0.05);观察组不良反应发生率为6.98%,与对照组16.28%相比无显著差异(P>0.05)。结论:养阴益气复元汤加减治疗慢性肺源性心脏病(肺肾气虚型)可提高患者临床疗效,改善患者血气指标,增强肺功能。Objective:To explore the curative effect of Modified Yangyin Yiqi Fuyuan Decoction for chronic cor pulmonale(lung-kidney Qi-deficiency type).Methods:A total of 86 patients with chronic cor pulmonale(lung-kidney Qi-deficiency type)admitted to the hospital were divided into an observation group(43 cases)and a controlled group(43 cases)by the random number table method between June 2019 and May 2021.The controlled group was treated with routine western medicine,while the observation group was additionally treated with Modified Yangyin Yiqi Fuyuan Decoction.All were continuously treated for 2 months.The curative effect between the two groups was compared.Before and after treatment,scores of TCM syndromes between the two groups were compared.The lung function indexes[forced expiratory volume in one second(FEV_(1)),forced vital capacity(FVC),FEV_(1)/FVC]were detected by spirometer.The partial pressure of oxygen(PaO_(2)),oxygen saturation(SaO_(2))and partial pressure of carbon dioxide(PaCO_(2))were detected by blood gas analyzer.The occurrence of adverse reactions during treatment in both groups was recorded.Results:The response rate of treatment in the observation group was significantly higher than that in the controlled group(90.70%vs 72.09%)(P<0.05).After treatment,scores of TCM syndromes in the observation group were significantly lower than those in the controlled group(P<0.05),FEV_(1),FVC,FVC/FEV_(1),PaO_(2),SaO_(2)and PaCO_(2)were significantly higher than those in the controlled group(P<0.05).There was no significant difference in incidence of adverse reactions between the observation group and the controlled group(6.98%vs 16.28%)(P>0.05).Conclusion:Modified Yangyin Yiqi Fuyuan Decoction can improve clinical curative effect and blood gas indexes,and enhance lung function in patients with chronic cor pulmonale(lung-kidney Qi-deficiency type).
