中药眼罩熏蒸联合睑板腺按摩在干眼症患者中的应用

摘　　要：目的:探讨中药眼罩熏蒸联合睑板腺按摩在干眼症患者中的应用效果。方法:将本院2020年3月至2021年5月期间收治的103例干眼症患者简单随机分为对照组(n=50)和观察组(n=53),对照组给予睑板腺按摩治疗,观察组在上述基础联合中药眼罩熏蒸治疗,连续治疗1个月,比较两组患者临床疗效,以及治疗前后的中医症候积分、干眼检查指标,统计复发率及不良反应。结果:观察组临床有效率(92.45%)较对照组(7.00%)显著增加,且差异有统计学意义(P<0.05);治疗前,两组患者干涩不适、异物感、白睛红赤、眼部疲劳症状积分比较无明显差异(P>0.05),治疗后,两组患者上述症状积分均下降,且观察组低于对照组(P<0.05);治疗前,两组患者泪液分泌量、泪膜破裂时间(BUT)、角膜荧光素染色(FL)评分比较不具备统计学意义(P>0.05),治疗后,两组患者泪液分泌量、BUT均升高,FL评分均下降,且观察组优于对照组(P<0.05);观察组复发率(1.89%)低于对照组(12.00%),且差异具有统计学意义(χ^(2)=4.154,P=0.041);两组患者均未发生不良反应。结论:干眼症患者应用中药眼罩熏蒸联合睑板腺按摩治疗,疗效确切,可有效改善临床症状,促进泪膜功能恢复,降低复发率,且安全性高。Objective:To investigate the application effect of traditional Chinese medicine(TCM)eyepatch fumigation combined with meibomian gland massage in patients with xerophthalmia.Methods:103 patients with xerophthalmia admitted to the hospital from March 2020 to May 2021 were randomly divided into a controlled group(n=50)and an observation group(n=53).All patients were treated with meibomian gland massage,and patients in the observation group were additionally treated with TCM eyepatch fumigation.Both groups were given 1 month of treatment.Clinical effect,TCM symptom scores and examination indexes of the dry eye before and after treatment were compared between the two groups.The recurrence rate and adverse reactions were counted.Results:The clinical response rate in the observation group(92.45%)was significantly higher than that in the controlled group(7.00%)(P<0.05).There was no significant difference in the scores of dryness and discomfort,foreign body sensation,chemosis and hyperemia of bulbar conjunctiva,and eye fatigue between the two groups before treatment(P>0.05).After treatment,the above symptom scores were reduced in the two groups,and the scores in the observation group were lower than those in the controlled group(P<0.05).There was no significant difference in tear secretion volume,break-up time of tear film(BUT)and corneal fluorescein staining(FL)score between the two groups before treatment(P>0.05).After treatment,tear secretion volume and BUT were increased,and FL score was reduced in the two groups.The three indicators in the observation group were better than those in the controlled group(P<0.05).The recurrence rate in the observation group(1.89%)was lower than that in the controlled group(12.00%)(χ^(2)=4.154,P=0.041).No adverse reactions were observed in the two groups.Conclusion:TCM eyepatch fumigation combined with meibomian gland massage is effective in the treatment of xerophthalmia,which can effectively improve clinical symptoms,promote the recovery of tear film function,and reduce the recurrenc
