虫半夏白术天麻汤联合丹红注射液对风痰瘀阻型急性缺血性脑卒中患者血液流变学及血清炎症指标的影响

摘　　要：目的:探讨虫半夏白术天麻汤联合丹红注射液对风痰瘀阻型急性缺血性脑卒中患者血液流变学及血清炎症指标的影响。方法:选取2018年1月~2020年1月河北省第七人民医院神经内科收治的100例急性缺血性脑卒中患者,按随机数字表法均分为对照组与观察组。两组均予以急性缺血性脑卒中常规治疗,对照组加用丹红注射液治疗,观察组在对照组基础上加用虫半夏白术天麻汤治疗。评估两组疗效和安全性。比较两组治疗前后中医证候积分、美国国立卫生研究元卒中量表(NHISS)评分。比较两组治疗前后血液流变学及血清炎症指标。结果:观察组患者总体治疗有效率显著高于对照组(P<0.05)。两组患者中医证候积分均明显降低(P<0.05),且观察组降低更为明显(P<0.05)。治疗后两组NIHSS评分、血液流变学指标及血清炎症指标均显著低于治疗前(P<0.05),且观察组患者NIHSS评分、血液流变学指标及血清炎症指标均显著低于对照组(P<0.05)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论:虫半夏白术天麻汤联合丹红注射液治疗风痰瘀阻型急性缺血性脑卒中临床疗效与安全性均较高,可有效改善患者临床症状,减轻炎性反应及促进血液流变学改变。Objective:To investigate the effect of Chongbanxia Baizhu Tianma Decoction combined with Danhong Injection on hemorheology and serum inflammatory indexes for patients with acute ischemic stroke of wind phlegm stasis type.Methods:A total of 100 patients with acute ischemic stroke in our hospital from January 2018 to January 2020 were randomly divided into a controlled group(n=50)and a study group(n=50).Both groups were given routine treatment of acute ischemic stroke,patients in the controlled group were treated with Danhong Injection,and patients in the study group were treated with Chongbanxia Baizhu Tianma Decoction on the basis of the controlled group.The efficacy and safety of the two groups were evaluated.TCM syndrome score and NHISS score were compared between the two groups.Results:The overall effective rate of the study group was significantly higher than that of the controlled group(P<0.05).The TCM syndrome scores of the two groups were significantly decreased(P<0.05),and the study group was more significantly lower than the controlled group(P<0.05).After treatment,NIHSS score,hemorheology index and serum inflammation index of the two groups were significantly lower than those before treatment(P<0.05),and NIHSS score,hemorheology index and serum inflammation index of the study group were significantly lower than those of the controlled group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The clinical efficacy and safety of using Chongbanxia Baizhu Tianma Decoction combined with Danhong Injection in the treatment of acute ischemic stroke of wind phlegm stasis type are high,which can effectively improve the clinical symptoms of patients,reduce the inflammatory reaction and promote the changes of hemorheology.
