银翘红酱汤合逍遥散治疗免疫性不孕临床疗效及对患者血清免疫学指标及安全性的临床对照研究

摘　　要：目的:探究银翘红酱汤合逍遥散治疗免疫性不孕患者的临床疗效、安全性及对血清免疫学指标的影响。方法:选取2019年4月~2020年10月西安市中医医院收治的78例免疫性不孕患者,随机数字表法分为对照组和观察组,各39例。对照组采用常规西药对症治疗,观察组在对照组基础上给予银翘红酱汤合逍遥散治疗,均治疗3个疗程。比较两组临床疗效、安全性,观察并记录两组血清免疫学指标变化。随访6个月,统计并比较两组治疗后的受孕率。结果:治疗后,观察组临床有效率为94.74%,显著高于对照组的72.97%(P<0.05);治疗期间,两组免疫性不孕患者均未出现严重肝、肾损伤,观察组安全性较好(10.53%vs29.73%)。治疗后,观察组抗精子抗体(AsAb)、抗子宫内膜抗体(EMAb)、抗卵巢抗体(AoAb)及抗心磷脂抗体(ACA)阳性率分别为5.26%、13.16%、10.53%、15.79%,均低于对照组24.32%、32.43%、29.73%、37.84%,差异均有统计学意义(P<0.05);随访6个月,观察组受孕率为92.11%(35/38),高于对照组受孕率67.57%(25/37)(P<0.05)。结论:银翘红酱汤合逍遥散治疗免疫性不孕临床效果显著,安全性更高,同时可改善血清免疫学指标转阴率,具有较好的推广价值。Objective:To explore the clinical efficacy and safety of using Yinqiao Hongjiang Decoction combined with Xiaoyao Powder in the treatment of patients with secondary infertility and its influence on serum immunological indicators.Methods:A selection of 78 patients with immunological infertility admitted to Xi’an Hospital of Traditional Chinese Medicine from April 2019 to October 2020 were randomly divided into a controlled group and an observation group,with 39 cases in each group:39 cases in the controlled group were treated with conventional western medicine.39 cases in the observation group were treated with Yinqiao Hongjiang Decoction and Xiaoyao Powder on the basis of the controlled group for a total of 3 courses.Compared the clinical efficacy and safety of the two groups,observed and recorded the changes in serum immunological indicators of the two groups.Follow-up for 6 months,statistics and comparison of the pregnancy rate of the two groups after treatment.Results:After treatment,the clinical effective rate(94.74%)of the observation group was higher than that of the controlled group(72.97%)(P<0.05).During the treatment period,neither two groups of patients with immune infertility had severe liver or kidney damage,and the observation group had better safety(10.53%vs.29.73%).After treatment,the positive rates of anti-sperm antibody(AsAb),anti-endometrial antibody(EMAb),anti-ovarian antibody(AoAb)and anti-cardiolipin antibody(ACA)in the observation group were 5.26%,13.16%,10.53%,and 15.79%,respectively,were lower than the controlled group by 24.32%,32.43%,29.73%,37.84%,the difference was statistically significant(P<0.05).Followed up for 6 months,the pregnancy rate of the observation group was 92.11%(35/38),which was higher than the controlled group by 67.57%(25/37)(P<0.05).Conclusion:Using Yinqiao Hongjiang Decoction and Xiaoyao Powder have significant clinical effects in the treatment of immune infertility,and have higher safety.At the same time,it can improve the negative rate of serum immunological indica
