摘 要:目的:应用中药油剂愈溃油外治糖尿病皮肤溃疡大鼠,探讨其对创面愈合及Wnt、notch通路的影响。方法:建立大鼠糖尿病皮肤溃疡模型并使用随机数字表法分为模型组、速甲舒组(阳性对照)、中药油剂愈溃油组,每组15只,另取15只普通皮肤溃疡(非糖尿病引起的皮肤损伤)大鼠作为对照组,使用生理盐水清洗大鼠创面后,中药油剂愈溃油组和速甲舒组分别于创面处涂抹适量对应药物并不予清除,对照组和模型组不涂抹药物,每天2次,连续16天。于给药期间第3 d、9 d、16 d,使用Image J软件观察并计算各组大鼠创面愈合率,酶联免疫吸附(ELISA)法检测各组大鼠血清中IL-6和TNF-α含量。于末次给药结束24 h后,苏木素-伊红(HE)法染色观察各组大鼠创面组织病理变化,免疫组织化学法检测各组大鼠创面组织中α-SMA和vWF表达水平,蛋白免疫印迹(western blot)法检测各组大鼠创面组织中Wnt、notch和β-catenin蛋白表达水平。结果:给药第9 d和16 d,与对照组相比,模型组创面愈合率显著降低,血清中IL-8和TNF-α含量显著升高(P<0.05),与模型组相比,中药油剂愈溃油组和速甲舒组创面愈合率显著升高,血清中IL-8和TNF-α含量显著降低(P<0.05)。末次给药后,与对照组相比,模型组大鼠创面皮肤表层较薄,成纤维细胞和新生毛细血管较少,有大量炎性细胞浸润,α-SMA、VW、Wnt、notch和β-catenin表达水平显著降低(P>0.05),与模型组相比,中药油剂愈溃油组和速甲舒组大鼠创面皮肤表层较厚,有大量成纤维细胞及新生毛细血管,炎性细胞浸润减少,α-SMA、VW、Wnt、notch和β-catenin表达水平显著升高(P>0.05);中药油剂愈溃油组和速甲舒组各项指标差异无统计学意义(P>0.05)。结论:中药油剂愈溃油可显著促进糖尿病皮肤溃疡大鼠的创面愈合,可能与激活Wnt、notch通路有关。Objective:To treat the diabetic ulcer rat model by external traditional Chinese medicine oiling agent Yukui Oil,and to investigate the effect of traditional Chinese medicine oiling agent Yukui Oil on wound healing and Wnt,Notch pathways for diabetic ulcer rats.Methods:After the establishment of diabetic skin ulcer model,random number table method was used to divided rats into a model group,a Sujiashu group(positive control)and a Chinese medicine oiling agent Yukui Oil group,with 15 in each group,and another 15 rats with normal skin ulcers(non-diabetic skin lesions)were selected as the controlled group.After the wounds were cleaned with normal saline,the Yukui Oil group and the Sujiashu group were smeared with appropriate amount of corresponding drugs without clearing,while the controlled group and the model group were not smeared with drugs,twice a day for 16 consecutive days.The wound healing rate was measured and calculated on the 3^(rd),9^(th)and 16^(th)day after administration by Image J software,and the levels of IL-6 and TNF-αin serum were detected by ELISA.At 24 hours after the last administration,hematoxylin eosin(HE)staining was used to detect the pathological changes of wound tissue,the expression levels ofα-SMA and vWF in wound tissue were detected by immunohistochemistry,the expression levels of Wnt,Notch andβ-catenin in wound tissue were detected by Western blot.Results:On the 9^(th)and 16^(th)day of administration,compared with those in the controlled group,the wound healing rate of the model group was significantly lower,and the contents of IL-8 and TNF-αin serum were significantly higher(P<0.05),compared with those in the model group,the wound healing rate of Chinese medicine oiling agent Yukui Oil group and the Sujiashu group was significantly higher,and the contents of IL-8 and TNF-αin serum were significantly lower(P<0.05).After the last administration,compared with those in the controlled group,the skin surface of the model group was thinner,the number of fibroblasts and new capillaries w
