六味地黄汤加味联合情志护理对老年糖尿病合并失眠疗效及机制研究

摘　　要：目的:观察情志护理联合加减六味地黄汤对老年糖尿病合并失眠的疗效及其作用机制。方法:选择我院2018年1月1日~2021年7月31日住院部收治的80例老年糖尿病合并失眠患者,根据不同治疗方法分为对照组40例,予单纯常规治疗及常规护理;观察组40例,在对照组基础上给与情志护理联合加减六味地黄汤。观察两组患者疗效、睡眠质量、睡眠参数、下丘脑-垂体-靶腺轴激素(TSH、TRH、GH、Cor、FT4)水平。结果:①观察组总有效率82.5%显著优于对照组65%(P<0.05);②两组患者睡眠质量各指标评分较前均显著降低(P<0.05),且观察组显著低于对照组(P<0.05);③两组患者入睡潜伏期、觉醒时间、觉醒次数较前均显著降低(P<0.05),且观察组显著低于对照组(P<0.05),两组患者总睡眠时间、睡眠效率较前均显著提高(P<0.05),且观察组显著高于对照组(P<0.05);④两组患者下丘脑-垂体-靶腺轴激素水平较前均显著降低(P<0.05),且观察组显著低于对照组(P<0.05)。结论:情志护理联合六味地黄汤加减对老年糖尿病合并失眠患者临床疗效显著,可有效控制血糖,改善失眠症状,这可能与调节患者下丘脑-垂体-靶腺轴激素表达有关。Objective:To explore the effect and mechanism of emotional nursing combined with Liuwei Dihuang Decoction on senile diabetes with insomnia.Medthods:80 elderly diabetes patients with insomnia were selected in our department from January 12018 to July 312021.They were divided into 2 groups according to different treatment methods,40 cases in the controlled group:simple routine treatment and routine nursing.40 cases in the observation group:add emotional nursing combined with Liuwei Dihuang Decoction on the basis of the controlled group.The curative effect,sleep quality,sleep parameters and the levels of hypothalamic pituitary target gland axis hormones(TSH,TRH,GH,Cor,FT4)were systematically observed.Results:(1)The total effective rate of 82.5%in the observation group was significantly better than 65%in the controlled group(P<0.05);(2)The scores of sleep quality indexes in the two groups were significantly lower than those in the previous group(P<0.05),and the scores in the observation group were significantly lower than those in the controlled group(P<0.05);(3)The sleep latency,wake-up time and wake-up times in the two groups were significantly lower than those in the previous group(P<0.05),and the total sleep time and sleep efficiency in the observation group were significantly higher than those in the controlled group(P<0.05);(4)The levels of hypothalamic pituitary target gland axis hormones in the two groups were significantly lower than those in the previous group(P<0.05),and the levels in the observation group were significantly lower than those in the controlled group(P<0.05).Conclusion:Emotional nursing combined with Liuwei Dihuang Decoction has significant clinical effect for elderly diabetes patients with insomnia.It can effectively control blood sugar and improve insomnia symptoms,which may be related to regulating the expression of hormones in hypothalamus pituitary target gland axis.
