通窍活血汤加味联合耳后注射甲泼尼龙琥珀酸钠对突发性耳聋患者的疗效及对载脂蛋白的影响

摘　　要：目的:探讨突发性耳聋(SD)患者应用通窍活血汤联合耳后注射甲泼尼龙琥珀酸钠的治疗效果及治疗前后载脂蛋白水平的表达变化。方法:将80例SD患者随机将其均分为观察组(n=40)和对照组(n=40),对照组在常规治疗的基础上于患者耳后局部注射甲泼尼龙琥珀酸钠0.5 mg,观察组在对照组的基础上予以通窍活血汤加减,比较两组患者的治疗效果及比较两组患者治疗前后的听觉脑干诱发电位(ABR)异常率、纯音测听(PTA)阈值和载脂蛋白(Apo-AI和Apo-B)水平变化情况。结果:观察组患者治疗总有效率为95.00%,高于对照组总有效率82.5%,两组间比较有统计学意义(χ~2=5.175,P=0.023)。治疗后两组患者Ⅰ、Ⅲ、Ⅴ波PL异常率、PTA阈值和Apo-B水平较治疗前明显下降,观察组下降更为明显(P<0.05),两组间比较具有统计学意义(P<0.05),而两组患者治疗后血清Apo-AI水平较治疗前无明显变化(P>0.05)。结论:通窍活血汤联合耳后注射甲泼尼龙琥珀酸钠治疗SD的效果更佳,能够更显著地改善PTA阈值水平,可能与改善患者血脂代谢状态有关。Objective:To investigate the effect of Tongqiao Huoxue Decoction combined with retroauricular injection of methylprednisolone sodium succinate on therapeutic efficacy and apolipoprotein for patients with sudden deafness(SD).Methods:80 patients with SD were randomly divided into an observation group(n=40)and a controlled group(n=40).The controlled group was locally injected with 0.5 mg Methylprednisolone Sodium Succinate behind the affected ear on the basis of routine treatment,where the observation group was added and subtracted with Tongqiao Huoxue Decoction on the basis of the controlled group.The treatment effects of the two groups were compared,and the abnormal rate of acoustic brainstem response(ABR),changes of pure tone audiometry(PTA)threshold and apolipoprotein(Apo-AI and Apo-B)levels before and after treatment was compared.Results:The total effective rate of the observation group was 95.00%,which was higher than 82.5%of the controlled group,where the comparison between the two groups was statistically significant(χ~2=5.175,P=0.023).After treatment,the abnormal rate of waveⅠ,ⅢandⅤPL,PTA threshold and Apo-B level in the two groups decreased significantly compared with those before treatment,and the decrease was more obvious in the observation group(P<0.05),where has significant difference between the two groups(P<0.05),but the serum Apo-AI level in the two groups had no significant change after treatment(P>0.05).Conclusion:Tongqiao Huoxue Decoction combined with retroauricular injection of Methylprednisolone Sodium Succinate is more effective in the treatment of SD,which can significantly improve the threshold level of PTA,which may be related to the improvement of blood lipid metabolism.
