黄柏阿胶汤治疗对溃疡性结肠炎患者内镜表现、肠粘膜屏障功能和血粘附分子的影响

摘　　要：目的:研究黄柏阿胶汤治疗对溃疡性结肠炎(UC)患者内镜表现、肠粘膜屏障功能和血粘附分子的影响。方法:将85例UC患者随机均分为治疗组(黄柏阿胶汤治疗)和对照组(常规西药治疗),比较两组治疗前后中医证候变化、内镜表现、肠粘膜屏障功能和血粘附分子的变化。结果:治疗后,两组肠镜表现积分比治疗前低(P<0.05);治疗组低于对照组(P<0.05)。治疗后,两组中医证候积分均明显降低(P<0.05),且治疗组降低幅度更大(P<0.05)。治疗后,两组外周血中二胺氧化酶(DAO)、D-乳酸和内毒素水平均显著低于治疗前,且治疗组低于对照组(P<0.05);治疗后,两组P-选择素(P-sel)和细胞间黏附分子-1(ICAM-1)水平均明显降低(P<0.05),且治疗组降低幅度比对照组大(P<0.05)。结论:黄柏阿胶汤治疗溃疡性结肠炎可减轻黏膜功能受损,减轻血粘附分子病变,改善患者肠粘膜屏障功能。Objective:To study the effect of Huangbai Ejiao Decoction treatment on endoscopic manifestations,intestinal mucosal barrier function and blood adhesion molecules for patients with ulcerative colitis(UC).Methods:85 patients with UC were randomly divided into a treatment group(Huangbo Ejiao Decoction treatment)and a controlled group(conventional western medicine treatment).The changes in TCM syndromes,endoscopic manifestations,intestinal mucosal barrier function and blood adhesion molecules were compared between the two groups before and after treatment.Results:After treatment,the colonoscopy scores of the two groups were lower than those before treatment(P<0.05),and the score of the treatment group was lower than that in the controlled group(P<0.05).After treatment,the scores of TCM syndromes in the two groups were significantly reduced(P<0.05),and the reductions were even greater in the treatment group(P<0.05).After treatment,the levels of peripheral blood diamine oxidase(DAO),D-lactic acid and endotoxin in the two groups were significantly lower than those before treatment,and the levels of the treatment group were lower than those of the controlled group(P<0.05).After treatment,the levels of P-selectin(P-sel)and intercellular adhesion molecule-1(ICAM-1)were significantly decreased in the two groups(P<0.05),and the decreases in the treatment group were greater than those in the controlled group(P<0.05).Conclusion:Huangbai Ejiao Decoction can relieve the mucosal function damage,reduce the pathological changes of blood adhesion molecules,and improve the intestinal mucosal barrier function of patients with ulcerative colitis.
